| Breast cancer is one of the common malignant tumour in women. , it "s severely endanger women s health. Surgical resection is the mainly treatment. Now surgery therapy of infiltration breast cancer is whole or partical breast dissection, and axillary lymphnode dissection. Axillary lymphnode metastasis is the powerful indication of prognosis in breast cancer, and axilary lymphnode dissection combine biopsy is the golden standard for the judgement of lymphnode metastasis. But complication of lymphnode dissection can not be ignored,it affect the life quality of patient post operation. Most of the breast cancer patients, especially primery breast cancer patients without axillary lymphnode metestasis, the lymph-node has normal function to prevent lymphnode metestasis, so there is dissuction about lymphnode dissection. Sentinel Lymph node biopsy make a new way for the solution of this qusetion. The concept and theory of sentinel lymph node are based on tumor metestasis in turn , it present the condition of lymphnode metestasis. By the way of microinjury operation to get the sentinel lymph node and make biopsy to distinuish the patient with or without lymphnode metestasis, that make lymphnode dissection more rationalize.ObjectiveMake sentinel lymph node biopsy of 60 cases with breast cancer during operation. First, try to find metestasis by HE stain, then for the negetive cases with AE1/AE3 to make serial section and Immunohistochemical stain to detect the lymphnode metatasis.Material and methodMaterial;60 cases with breast cancer are selected in case group from 2001. 10 -2004.3 in Liaoning provincal hospital , except 2 cases received tumor dissection and axillary lymphnode dissection, other 58 cases received improving dis-sectiong operation. All the 60 cases were detected by methylene blue , made Sentinel Lymph node biopsy first ,then received axillary lymphnode dissection. The lymphnode 966 totally, sentinel lymph node are 120.Method: For the all cases with breast cancer , Both the 120 sentinel lymph node and the 846 non - sentinel lymph node are selected to make single section HE stain ,to make serial section at the level of l00um and make Immunohisto-chemical stain at the same time. Compare two methods with Pearson x2 , software SPSS10. 0.ResultsIn the 60 cases with breast cancer , get all 966 lymph node, Sentinel Lymph node 120 ,non - Sentinel Lymph node 846. With single section HE stain positive ,Sentinel Lymph node metastases 32(26.7% ), non - sentinel lymph node metastasis 37(4.4% ) . For the rest of the 88 (44cases) which HE stain negetive , make serial section HE stain and Immunohistochemical stain find another 16 (7case) sentinel lymph node micrometastasis. By the method of single section HE stain sentinel lymph node metastasis ratio is 26.7% (32/120) ,6.1 times of non - sentinel lymph node metastasis . But by the method of serial section HE stain and Immunohistochemical stain metastasis ratio of sentinel lymph node is 40% ,6. 0 times of non - sentinel lymph node metastasis (40/6. 7). The two methods have stastics significance (P <0.05). Compare serial section and Im-muno histo chemical stain with single section HE stain ,the detection senstivity improve 13. 3% (16/120).DisuctionSentinel lymph node is the first aim of tumour cell metatasis. Different methods were used to detect sentinel lymph node , single stain and GDP or lym-phy tree and also some with 2or3 methods. Stain and radiolabeled material were injected into subcutaneous surrounding the tumor inside tumor . In the last 10 years sentinel lymph node biopsy also used in the staging of cancer. , it's safety, sensetive and effective, also can show the axillary lymph node metastasis. Serial section combine with Immunohistochemical stain is helpful for detection of mi-crometastasis.ConclusionSLN biopsy is very importment for decision of axillary lymphnode dissection.Compare serial section HE stain and Immuno histo chemical stain with normal HE stain, detection senstivityfor SLN significantly improved.
