The Use Of Intravascular Ultrasound In Coronary Artery Disease Patients With Percutaneous Cornary Interventions

Objectives The values of intravascular ultrasound(IVUS) to diagnosis coronary artery disease(CAD) compared with coronary angiography(CAG) and the relation between atherosclerosis and remodeling of coronary artery and the guidance and the evaluation effect of IVUS on percutaneous cornary interventions were studied using IVUS.Methods The 152 coronary segments of 106 CAD with CAG pre-stenting operation,67 segments post-stenting and 21 segments after high-pressure ballon re-expansion were studied using IVUS. Results IVUS was more sensitive for visualizing the shape and transmural components of the artery wall and characteristics of atherosclerotic plaques than CAG.The Severity of coronary atherosclerosis was underestimated by CAG.Vessle remodeling was found in mild,moderate and severe vessel stenosis.There were significant differences among them.The vessel sectional area was significantly larger in the soft plaque vessels than that in the fibrotic and calcium ones.A significant vessel remodeling was seen in the eccentric plaques comparing with the concertric one. 46/67 segments indicated for stenting by IVUS had good results after stenting with IVUS and 21/67 segments with noideal results had good results checked with IVUS after high-pressure ballon re-expansion. Statistic analysis showed VA and LA were increased post-stenting.PA and area stenosis rate(ASR)were reduced. There was no difference between VA and PA,but LA and ASR had obvious difference between pre-high pressure and post-high pressure ballon re-expansion. Conclusion The sensitivity and the specificity of the IVUS was be superior to CAG. Vessel remodeling is apparently seen in the eccentric plaque and soft plaque.IVUS can be used to indicate stenting exactly. The vessels with bad stenting results can be improved by high-pressure ballon re-expansion.IVUS was an important and useful tool to assess the results of intracoronary stent in the patients with CAD.Chen Baoxia(internal medicine cardiovascular) Directed by Cai Shanglang Liu Chuanmu...
