OBJECTIVE Constipation is a common symptom lowering human being's quality of life. The prevalence of constipation is 5-20% in western world and incidence rate of elder men over 60 years old in our country is 10-25%. But it is unavailable in ordinary population. The aim of this study was to understudies the epidemiological feature and risk-factors of chronic constipation and analysis of parmacotherapy of PEG4000. METHOD 1). To investigate the prevalence of chronic constipation (CC) among adults in Beijing area , to identify the risk factors of CC and to analyze the relationship between CC and symptoms of upper gastrointestinal dysmotility using stratified randomized program by clustering sampling for CC. This study was carried out in 2486 subjects of inhabitants in Beijing area according to symptoms using CC criteria. The range of age was 18-70. All subjects were investigated by questionnaire during visiting to their families. Potential risk factors and symptoms of upper gastrointestinal dysmotility were investigated between CC and non-CC group using x2 and logistic analysis of multi-factors.2). To validate the effect, adaptive condition, safety and economic index of PEG4000 (polyethylene glycol) on adult chronic functional constipation by a multicenter random control clinical trail involved in PUMC, Shanghai Renji Hospital and Guangzhou Zhongshan Hospital. Totally we arehave observed 187 patients who from 3 centers, 93 in control group, and 94 in treatment group. All patients are aged between 18-70 years, suffering from chronic constipation for more than 6 months and without any colonic organic disease confirmed by colonoscope or barium enema examination. Both of the groups to be treated four weeks.RESULT (1)The prevalence of CC was 6.07% in Beijing area, 4.65% and 7.05% in the urban and rural; 2.11% and 9.68% in the male and female group, respectively (p<0.001). The prevalence of CC in the group of age 30-39 is the highest of all ages (7.67%, p=0.009),The multivariable analysis showed cultural level under junior middle school (OR=7.704, 1.134-52.344, p=0.037), psychological nervous (OR=7.286, 1.092-48.633, p=0.040), abnormal emotion (OR=6.573, 1.043-41,443, P=0.045), often drinking strong tea (OR=2.001, 1.070-3.740, p=0.030), smoking women with more than 10 cigarettes per day (OR=4.81, 2.15-10.77, p=0.001) were risk factors (p<0.0l). There are 17.22% (p<0.001) CC patients associated with gastroesophgeal reflux (GER) and 43.46% (p<0.001) with dyspepsia symptoms.(2) The result show that there is no significant difference between two groups on the significant effective rate and the total effective rate either 2 weeks or 4 weeks. The total effective rate of PEG4000 is 95.95%, and that of Duphalac is 94.52%.(p=0.685). Two groups to be treated after 4 weeks, the mean ejection rate of Barium Strip in GITT of the STC patients is 94.92%, significantly higher than 61.11% of the OOC patients (p=0.001). In PEG4000 group , the remission rate of abdominal distention is 60.29% for 2 weeks and 76.47% for 4 weeks, significantly higher than that of Duphalac group, 31.51% for 2 weeks and 43.86% for 4 weeks (p<0.001). The remission rate ofabdominalgia is the same so (p=0.025,0.026). But the remission rate of anepithymia of Duphalac group is 71.67% for 2 weeks and 86.67% 4 weeks, is significantly higher than PGE4000 group, 52.63% for 2 weeks and 70.18% for 4 weeks (p=0.034, 0.030). The fifth week there are more patients desire to elect PEG4000 (90.54%) ,is significantly higher than Duphalac (69.86%). The mean cost for daily treatment in 4 weeks , the PGE4000 group (5.89± l.86yuan) is significantly lower than Duphalac (8.21±1.17yuan, p=0.047). CONCLUSION (l).The prevalence of CC is 6.07%, and it is more than 4 times in female to male, and it is 1.5 times in urban to rural, and it is higher in the group of age 30-39(7.67%) in Beijing area.
