Objective: To investigate the association between psychosocial factor and episode and severity degree of diabetes mellitus(DM).Method: Eighty patients with DM and eighty normal control were involved in the study. The data of two group have no significant difference by balance test. All the subjective themselves completed the following questionnaires: general condition questionnaire, TABQ, LES, SSRS and DSQ. However , HAMD and HAMA were rated by the researcher. Plasm cortisol, growth hormtone, platelet CD62p, Plasm CD62p, platelet membran fibrinogen Binding response, TXB2, FPG and HbAlc were detected. Utilizing t test, x2 test and stepwise regression analysis analysed the collected data in SPSS 10.0.Results: (1) The ratio of TABP in DM patients is higher than normal controls, and there are positive correlations between the ratio of TABP and the severity degree of DM. Also, hostility score in major DM patients is higher than other DM patients and normal controls. (2) The incidence rate of depression and anxiety in DM patients is higher than controls. Furthermore, in depressive DM patients, their blood glucose control is worse, using immature defense style is more, social support is less, anxiety degree is higher ,growth hormtone level is higher and economic conditions are worse than controls. (3) The immature defense style score is higher in DM patients, also, the worse control of blood glucose is related to immature defense mechanism. (4) The worse controlof blood glucose in DM patients is related to less social support. (5) the intensity and frequency of life events is higher than controls. (6) DM patients with A type behavior, depression or anxiety and major DM patients have higher level of platelet activation than controls.Conclusions: (1) The severity degree of DM is related to TABP, depression, anxiety, immature defense style, less social support and life stress in DM patients. (2) Psychosocial facters may induce and worsen DM through neuroendocrine alternation and platelet activation mechanisms.
