Bladder cyst cancer is one of the most common urinary tumors , the mechanism of which is still unknown. Bladder cyst cancer has an obvious tendency of reoccurence . Recent statistics showed that the occurrence rate of bladder cyst cancer increased these years. The aim of tumor therapy is not only to cure primary cancer but to reduce tumor reoccurence. Tumor treatments available are still unsatisfied now. Progresses have been obtained recently in terms of tumor occurrence, development and prevention, and more attention has focused on the relationship between cell apoptosis and tumor development . Apoptosis , a specially procedured cell death , has become the focus in the field of clinical on-cotology .Apoptosis inhibitor survivin , also called survivor , is regarded the new member of IAP . Studies demonstrated that survivin , which might inhibit cell apoptosis , regulate cell cycle and participate angiogenesis, closely related to tumor occurrence , development and prognosis as a specific tumor marker. Resent researches focused on this subject. The conclusions of recent researches on survivin expression in liver cancer and breast cancer suggested that survivin gene only express in embryonic tissues and thymus , but never in normal mature tissues . However, excessive expression of survivin mRN A and its antigen could be found in all sorts of tumor tissues , the expression intensity of which related to the tumor grade and clinic stage of tumor. So survivin might become an ideal tumor target antigen in the process of immnodiagnosis and immnotherapy oftumor and an effective quota for prognosis and follow - up. Bladder cyst cancer is susceptible to reoccur. The characteristic of survivin prove to be an useful quota for immnodiagnosis, immnotherapy , prognosis and follow - up of bladder cyst cancer. While recent researches on expression of survivin in TCC is few o-verseas , and less in China . Hence , systemic research on survivin expression in normal bladder tissue and TCC , as well as research on the relatioship between survivin expression and cell apoptosis, proliferation, and cyclinDl expression , are very important to illuminate the mechanism of bladder cyst cancer development , to determine prognosis and to apply to immunodiagnosis and immnotherapy .ObjectiveTo study the correlation of the expression of survivin with the expression of ki -67, cyclinDl and cell apoptosis, this research tested the expression of survivin , ki - 67, cyclinDl in normal tissue and TCC by immunohistochemical method(IHC) and numbered the cells of apoptosis by Tunel method. More discussion was given on the role of survivin in TCC occurrence and development and cell apoptosis and proliferation, supplying a theoretical support to the applied value of survivin test in immunodiagnosis and immunotherapy of TCC.Methods1. Specimens which selected from tissue of operation and biopsy in the First Subordinate Hospital of Chinese Medical University from Jun 1997 to May 2003 were paraffined and preserved by the Department of Pathology. After being sliced for the thickness of 4um, HE dyeing was applied again for verification of pathological diagnosis by expert.2. 10 cases of normal bladder tissue and 46 cases of TCC were tested. The expressions of survivin, ki - 67 and cyclinDl in normal tissues and TCC were tested by IHC and the number of apoptosis cell by Tunel method. The correlativ-ity between the expression of survivin with that of ki - 67, cyclinDl and the cellapoptosis were studied by statistical method.Results1 Survivin does not express in the normal bladder mucus. The mean rate of survivin expression in 46 cases of TCC was 82. 6% , the expression index was 63.7 ; with an obvious difference (P <0.001) . The expression rate and expression index positively related to tumor grade and clinic stage of tumor .
