Cystoscope and urinary cytology examinations are the basic methods of bladder cancer's diagnosis. Cystoscope examination is traumatic and the sensitivity of urinary cytology examination is low. A important task is to search for a new method. Cytokeratin 20 is a new bladder cancer biomarkers(BCBs),the study of Cytokeratin 20 is one of hotspots at present. AIM To study the expression of CK20 in exfoliated urothelial cells of patients with bladder cancer and to evaluate it's clinical significane . METHODS Reverse transcriptase-polymerase chain reaction (RT-PCR) methods was used to determine the expression of CK20 in exfoliated urothelial cells of 45 patients with bladder cancer and 18 patients with nonmalignant disease in test one. Fluorescence quantify- polymerase chain reaction (FQ-PCR) methods was used to determine the expression of CK20 in exfoliated urothelial cells of 67 patients with bladder cancer in test two.GAPDH was used as internal control. RT-PCR methods was used to determine the expression of CK20 in exfoliated urothelial cells of 139 patients who underwent transurethral resection of bladder tumors or partbladder resection. RESULTS In test one,of 45 patients with bladder cancer ,39 specimen were positive and 1 of 18 patients with nonmalignant disease was positive. The sensitivity of RT-PCR method was found to be 86.7%,and the specificity was 94.4%. CK20 was positive in 24 of 29 G1 patients, in 11 of 12 62 patients and in 4 of 4 G3 patients .CK20 was not related to the tumor grade and stage.In test two, the expression quantity of CK20 in high grade and stage tumor were higher than those in low grade and stage tumor. There was significant difference between different grade and stage tumor. There was no difference in GAPDH among these groups. In test three,43 of 139 patients with bladder cancer recurred in following-up course. 45 of 139 patients's CK20 were positive.38 of these patients went with tumor's recurrence. 3 patients whose CK20 were positive were not found tumor's recurrence.but were found tumor's recurrence 3 or 6 months later. 4 patients whose CK20 were positive were not found tumor's recurrence by now. 2 patients whose CK20 were negative were found tumor's recurrence. CONCLUSION CK20 may be a good tumor biomarker for bladder cancer. RT-PCR method had higher sensitivity and specificity, the expression quantity of CK20 was significant difference between different grade and stage tumor.whether it can replace cystoscope in diagnosis and post-operation's inspection should ulteriorly study.
