Objectives To describe the prevalence of women of childbearing age with mental deficiency and their mal-pregnant outcomes in an administrative town of Anhui province, and analysis the impairments on their reproductive health. To investigate the serum thyroid stimulating hormone ( TSH ) level and reproductive health impairments of women with mental retardation ( MD ) in the rural area, therefore, provide the scientific evidences for women with MD to promote reproductive health and improve the population qualities.Methods The survey on the prevalence of women of child-bearing age with mental deficiency aged 20 to 49 years and their mal-pregnant outcomes were proceeded group by group in an administrative town of Anhui province, and all the clues was provided by the officer, epidemic prevention members and village people group's leaders. 92 women aged 20 to 49 with MD were screened by the Adult Intellecture Deficiency Scale and diagnosised by WAIS-RC. Among them, 74 were married and in childbearing age. 20 were caused by various encephalitis, 54 were caused by other unkown reasons. The health condition of women with MD was investigated and the prevelence of mental deficiency was calculated. The mal-pregnant outcomes of women with MD caused by various encephalitis and other unknow reasons was surveyed and analysised.148 normal women aged no more 3 years than the women with MD in the same administrative countryside were selected as the control group at the same time. Both of the two groups women's age, career, educational background, husbands, the living condition and the reproductive health were investigated.The cut-off point of sub-clinical hypothyroidism was defined as more and equal to 4mU/L of serum TSH level. The serum TSH level of 315 women with MD of unknown reasons, 117 MD women caused by certain reasons, and 546 women without MD wereassayed by the time resolved fluoroimmunoassay. Sixty women were selected randomly as the internal control group and the extranal control group respectively, matching with women of TSH 4mU/L by the age no more than 3 years. The living condition and the reproductive heath of three groups of women were investigated and comparised. Results Among 11 856 women of child-bearing age who were accepted the investigation, 92 women were evaluated as mental deficiency, and the prevelence of women with MD was 7.8%. The difference of the prevelence of women with MD between age groups was not significant (P=0.206). Among 92 women with MD, 16.3% had epileptic seizures, 3.3% had congenital heart disease. 11.3% had audio or visual disorders, 7.6% had epipod and lower limb disabilities. Among the levers of IQ evaluated by WAIS-RC, 45.7% were light degree, the secondary and severity degree were 23.9% respectively. The most severity degree was 6.5%. The risk of children with MD whose mother was MD with unknown reasons was increased (P =0.007). The differences of the mal-pregnant outcome between women with MD caused by encephalitis and unknow reason was not significant.Most of the women with MD hadn't received the regular schooling. 31.3% could not take part in the labor. The living condition of them was poor. The light and the middle degree of the scores of the Adult Intellecture Deficiency Scales were dominant. The age as their husbands married were older than the control groups (P=0.000), but the difference of the married age of women was not signficant. The distance between the age of husbands and women with MD was longer than the control group, so did the time between married and pregnanted. The difference of the health condition, the mal-conditions in the pregnance, diseases in the pregnance, children with neonatal diseases and birth defects between the case and the control groups were significant. Non-conditional multiple Logistic regressions showed that the variable of the house tipe, the age and educational backgrouds of husbands were risk factors of the reproductive health imparements in women with MD.
