Objective: To campare the levels of CEA, CYFRA21-1, NSE and ET-1 in blood and bronchoalveolar lavage fluid among patients with peripheral type lung carcinoma, benign lung disease and healthy controls. The aim of this study is to evaluate the value of serum indicators measured together both in blood and bronchoalveolar lavage fluid for early diagnosis of peripheral type lung carcinoma. Method : Fifty-six cases of peripheral type lung carcinoma, thirty cases of benign lung disease and thirty-three cases of healthy controls were studied. Blood samples were taken from all of the three groups at admission. The first two groups' bronchoalveolar lavage fluid were gathered. Levels of CEA , CYFRA21-, NSE and ET-1 in blood and bronchoalveolar lavage fluid were measured by radioimmunoassay method. Results: Whether in blood or in bronchoalveolar lavage fluid, the levels of CEA, CYFRA21-, NSE and ET-1 in cases of peripheral type lung carcinoma were showed significantly higher than that in cases of benign lung disease and healthy controls ( P<0. 05 ) . In NSCLC groups, the levels of CEA, CYFRA21-1 and ET-1 in blood were higher than that in SCLC group ( P<0. 05 ) , While NSE was higher in SCLC group than that in NSCLC groups( P<0. 05 )-The levels of CEA and CYFRA21-1 in blood between groups of adenocarcinoma and squamous cell carcinoma had differences ( P<0. 05 ) . When measured singly, CEA was more sensitive for adenocarcinoma, CYFRA21-1 forsquamous cell carcinoma, NSE for SCLC, ET-1 for NSCLC respectively. There would be a higher expression of the four indicators in a later stage of carcinoma ( P<0. 05 ). When the four indicators were measured together, the positive rate for adenocarcinoma and squamous cell carcinoma increased dramatically, reaching 91.7%, 92.3% respectively. Using two indicators, the most sensitive combination for adenocarcinoma and squamous cell carcinoma were CEA plus CYFRA21-1. The sensitivities of four indicators measured in bronchoalveolar lavage fluid were higher than that in blood, but the specificities were lower than that in blood. So when measured together, we had even higher sensitivities and specificities. Conclusion: These findings suggest that a combination of CEA CYFRA21-K NSE and ET-1 assay in blood and bronchoalveolar lavage fluid may be a useful measurement in early diagnosis of peripheral type lung carcinoma.
