purpose: To study the role of peroxisome proliferator-activated receptor alpha (PPAR-a), the therapeutic potency of PPAR-a ligands and the effect of compound traditional Chinese medicine in nonalcoholic fatty liver disease. methods: 46 Wistar rats,control group(n=8), fed normal diet; 38 rats, fed high fat diet.Four of thirty-eight rats were randomly sacrificed after four weeks to be identified the model of nonalcoholic fatty liver. 32 rats were randomly divided into 4 groups: model group(n=10); traditional Chinese medicine high dose group(n=8); traditional Chinese medicine low dose group(n=8); gemfibrozil group(n=8). All rats were sacrificed by cervical dislocation after 8 weeks. We examined the effect of traditional Chinese medicine on body weight, liver weight, alanine aminotransferase(ALT), aspartate aminotransferase(AST), total cholesterol (TC), triglycerides-(TG), free fatty acid(FFA) in serum and hepatic tissue and the expression of PPAR-a target genes encoding enzyme acyl-CoA oxidase(AOX) by RT-PCR.Results: 1. lipids:The mean of TC, TG, FFA in serum and hepatic tissue were significantly higher in model group than that in control group(P<0.05), were significantly lower in three treatment groups than that in model group(P<0.05). The mean of FFA in hepatic tissue and FFA, TC, TG in serum were no significantly different between traditional Chinese medicine high dose group and gemfibrozil group(P>0.05).2. ALT, AST: the mean of ALT, AST in serum were remarkably higher in model group than that in control group (P<0.05), were remarkably higher in gem group than that in traditional Chinese medicine high dose group(P<0.05), the difference was no significant between control group and traditional Chinese medicinehigh dose group(P>0.05).3. AOXmRNArthe expression of AOXmRNA was higher in three treatment groups than that in model group(P<0.05), was remarkably higher in gemfibrozil group than that in traditional Chinese medicine group(P<0.05).4. histology: The model group have a remarkable fatty liver, steatohepatitis. The level of steatosis and steatohepatitis were significantly lower in traditional Chinese medicine high dose group and gem group than that in model group(P<0.05). Hydroncus, degeneration, necrosis of hepatic cell in gem group were more obvious than others. Conclusion: 1. The effect of compound traditional Chinese medicine is remarkable in nonalcoholic fatty liver disease.2. Gemfibrozil that increase ALT and AST is harmful to liver, while compound traditional Chinese medicine is no harmful .3. PPAR-a and AOX have a beneficial compensatory effect in NAFLD.4. The ligands of PPAR-ais prospective in the treatment of nonalcoholic fatty liver disease..
