Obsjective: Spinal cord injury is a common cause of neuropathic voiding dysfunction. Neuropathic voiding dysfunction often cause pyelectasis, hydronephrosis, urinary system infection, finally leading to function damage or leasa of kidney. Renal failure is the main fatal reason of these patients. At present there is no ideal therapeutic method to these kinds of diseases. The change of function and nerve situation of the bladder is controversial and demands deeper realization after spinal cord injury at different level. Dog's neurogenic bladder model were established by transecting the spinal cord at different level. We observe the change of urodynamics and neuropeptide Y (NPY), nitric oxid synthase(NOS) in the bladder, thereby investigate the type of neurogenic bladder and bladder regulation mechanism of voiding dysfunction at different level of spinal cord injury(SCI).Methods1 Animal model preparation1.1 Animal groups distribution: choose 14 healthy adult dogs, female totally, divided randomly into control group (n=2); T8-9 spinal cord transection group(experimental group A,n=4);L1-2 spinal cord transection group(experimental group B,n=4);L7S1 spinal cord transection group(experimental group C,n=4), no marked weight difference after grouping, begin to make model after feeding one week.1.2 The establishment of modelUsing intravenous anesthesia (pentothal sodium 25mg/kg), then resecting lamina of vertebra with bone ribbing rongeur and exposing spinal cord, finally transecting spinal cord with lance, stanching with gelatin sponge, suturing incisions. Only resecting Lamina of vertebra in control group. Every dog was injected with Saline and 10% glucose 500ml per day for one week.2 Measurement content2.1 Spinal cord function observation: measure the function of lower limbs and cauda paraplegia after SCI, based on shaodingxu's six-grade lower limbs praxiology.2.2 Urodynamic examination: measure the change of intravesical pressure, cystometric bladder capacity and reflective urination. Urodynamic examinations were performed to each group one day before and one day after operation. Experimental groups were performed urodynamic examinations again after 3 weeks. Simultaneously observed the urination of dogs.2.3 Histological observation of bladder wall: 3 weeks after transection, neurogenic bladder model was proved to be successful by urodynamic examination. executed animal and then took out bladder immediately , 1.0×1.0cm full thickness bladder samples were taken from dome and trigone of bladder. Samples were fixed in 4% paraformaldehyde for 24 hours, after that embedded in paraffin. Observed morphologic change in HE staining; Observed change of nerve transmitter such as NOS, NPY in bladder myenteric plexus using immunohistochemical staining, making half-quantitative analysis, positive nerves in 20 eyeshots were magnified by 10 times in optical microscope and their surface density were analyzed using CMIAS poly-function pathological image analysis system.2.4 Data processing: This experiment's measure data is expressed by xˉ±s, adopting SAS6.12 stat software, choosing two-group one-way ANOVA, test levelα=0.05;Result of urodynamic analysis , surface density result of NOS, NPY-positive nerve immunohistochemistry in bladder myenteric plexus, offer total data, adopt SAS6.12 stat software, choose two-group one-way ANOVA, test levelα=0.05;Half quantitative result of NOS,NPY-positive nerve immunohistochemistry in bladder myenteric plexus, test method is two-group multiple-samples rank sum test(Kruskal-Wallis Test)and multiple comparison(Nemenyi Test),test levelα=0.05Results1 Spinal cord function : The spinal cord function of dogs in experimental group A and B are all classⅠ, in experimental group C are classⅠ(n=2) and classⅡ(n=2) .Dogs in experimental groups are all paraplegia and can't recover 21days after transection.
