Objective To evaluate the value of parotid gland ultrasonography as a diagnosti tool in second Sj?gren's syndrome. Methods Parotid gland ultrasonography was studied in 23 patients with Rheumatoid Arthritis and Sj?gren's syndrome(patients group.PG),26 controls with only Rheumatoid Arthritis(patients control group,PCG),and 25 controls with healthy(healthy control group,HCG). Firstly, parotid gland duct sialography,Schirmer's test,salivary flow rate, parotid gland scintigraphy and anti-SSA and /or anti-SSB antibodies was carried out.And without any cureparenchymal echo homogeneity of parotid gland, parenchymal echo quality of parotid gland,thickness of parotid gland and main duct of parotid gland were evalued by two-dimension ultrasound. Systolic peak flow velocity(SPFV) ,diastolic peak flow velocity(DPFV), resistance index(RI),pulse index(PI) was evaluated by color doppler ultrasound.Results The positive rate of parotid gland duct sialography, Schirmer's test,parotid gland scinitigraphy and anti-SSA and /or anti-SSB antibodies is siginificantly higher in PG than that in PCG and HCG and salivary flow rate is siginificantly lower in PG than that in PCG and HCG. The rate of parenchymal echo inhomogeneity and decreased parenchymal echo is siginificantly higher in PG than that in PCG and HCG,the thickness of parotid gland is siginificantly thicker in PG than that in PCG and HCG and The appearance rate main duct of parotid gland is siginificantly lower in PG than that in PCG and HCG.SPFV is siginificantly higher in PG than that in PCG and HCG,RI is siginificantly lower in PG than that in PCG and HCG.There is no statistically significant difference in DPFV and PI in PG to PCG and HCG. There is no significant difference in all guidelines between PCG and HCG.Conclusion Parotid ultrasonography is a very useful method for diagnosis of the parotid gland component of Rheumatoid Arthritis and Sj?gren's syndrome.We can diagnose accurately by evaluating thickness of parotid gland,parenchymal echo homogeneity of parotid gland, parenchymal echo quality of parotid gland ,main duct of parotid gland, SPFV and RI and contacting clinical datas including parotid gland duct sialography,Schirmer's test,salivary flow rate, parotid gland scintigraphy and anti-SSA and /or anti-SSB antibodies.And it can be used as a first diagnostic step in suspected cases of Rheumatoid Arthritis and Sj?gren's syndrome and as a monitoring application during the follow-up of Rheumatoid Arthritis and Sj?gren's syndrome patients.
