With the increasing requirements of health care, the mushroom pharmaceutical expenditure has been a heavy burden, which hinders the improvement of health care in the world. As for the principle of drug selection, besides efficiency and safety, the expenditure of drug therapy has been paid much attention to in the clinicaltherapeutically determination and proper drug use.Pharmacoeconomics is a science of applied methods of modern economics,combining with phamacoepidemology, decision and biostatistics, analyzing cost, benefit or effectiveness of different drugs therapy , and comparing with economic distinction. Main tasks of Pharmacoeconomics are to identify, measure and compare relative ratio of economics by different drugs and medical intervention (such as operation). It can help doctors select best cost-effective drugs and make sufficient use of limited health resources and avoid misspend.Peripheral facial paralysis is an acute facial damage with uncertain pathogeny, and not suppurative, which implicates facial function, such as facial asymmetry when smiling, even tic, having trouble in closing eyelid, corner of the mouth prolapse, etc. Epidemiology investigation shows that yearly incidence is 42.5 per 100 thousand people in our country, higher than 20 per 100 thousand people of Occident. Up to now, the main therapy methods are medicine cure or cooperated physical therapy.Methylcobalamin and prednisone are two drugs, which are usually used in the treatment of peripheral facial paralysis now. Methylcobalamin is a ramification of Vitamin B12 (Cyanocobalamin). Prednisone is a cortex hormone medicine. But the price of these two drugs is six to one. So it is necessary to adopt the theories and methods of pharmacoeconomics to investigate the cost and clinic comprehensive effects of the two drugs.Main conclusion as follows:(1)Based on 130 cases by diagnostic standard of facial paralysis, there are two cure groups, 71 cases of which are distributed to c, and 59 cases of which are distributed to prednisone group by randomization. After mensuration, we got the clinic facial paralysis grade from specimen, the average grade of methylcobalamin group is 35.48 while prednisone group's is 33.79, and we set paralysis to normal as 0 to 100. So the patients suffered middle or heavy facial paralysis, and there is little difference between two groups (t>0.05).(2)Clinic therapeutic effect: methylcobalamin group was in hospital for 18.47 + 7.02 days, while prednisone group was there for 20.66 + 6.86 days, t>0.05. Compared with respectively clinic effects, methylcobalamin group got 88.25 +9.96 grade, is better than what prednisone group got, 62.97 + 10.09, P<0.01.(3)Cost analysis: the average expenditure of each group took is Y441.34 +187.68 for methylcobalamin group, Y33.12+57.23 for prednisone group, P<0.01. As for the average total charge, methylcobalamin group cost Y 3111.97 + 1154.58 and prednisone group cost Y3348.67+ 853.22, P>0.05. It reveals that unit price of methylcobalamin group is much more expensive than that of prednisone group, but there is no remarkable difference of two groups on the total charge.(4) Cost/Effect analysis: the ratio of methylcobalamin group is 3111.97/ (88.25-35.48)=58.97, which means it took Y 58.97 to get one grade of facial mensuration averagely. The ratio of prednisone group is 3348.67/(62.97-33.79) =114.76. So methylcobalamin is much more effective than prednisone.(5)Sensitivity analysis: By charge: to add 15% total charge of methylcobalamin group, its cost/effect ratio is 3578.77/(88.25-35.48)=67.82, which is still better than prednisone group. To reduce 15% total charge of prednisone group, its ratio is 2846.37/ (62.97-33.79)=97.55, which is still worse than methylcobalamin group.By effect: to debase 15% effect of methylcobalamin group and improve 15% effect of prednisone group, the ratio of each group is 3111.97/(75.01-35.48)=78.72, and 3348.67/(72.41-33.79)=86.71 respectively.Conclusion and suggestion: this study is to prove that methylcobalamin is more effec...
