Objective The various bone (periosteal) flaps of ilium pedicled with vessel were applied in clinic .But the viewpoint of how to select the optimal operation mode is different. So we studied the character of all vessels that supplying blood to anterior iliac crest by systematic dissection,to provide anatomical basis for clinical rational application. Methods The origin, course, branches, distribrtion, outer diameter and anastomosis of the vessels of anterior iliac crest were observed on 32 adult cadavers perfused with red latex. At the same time,we measured the position of body surface projection of the vessels' original point, and studied the rotation range. Result There is abundant blood supply on anterior iliac crest,it is composed of four blood vessels.Superficial circumflex iliac artery, the outer diameter of the original point is (1.5 0.3)mm,the distance from the original point to the anterior superior iliac spine is (6.0 l.l)cm. Deep iliac circumflex artery, the outer diameter of the original point is (2.7 0.5)mm,the distance from the original point to the anterior superior iliac spine is (4.3 0.7)cm. Ascending branch of lateral circumflex femoral artery, the outer diameter of the original point is (2.5 0.4)mm,the distance from the original point to the anterior srperior iliac spine is (10.0 0.8)cm.Deeply superior branch of superior gluteal artery, the outer diameter of the original point is (2.7 0.6)mm,the distance from the original point to the top of iliac crest is (8.0 0.6)cm.each vessel has its character, and they have great complementation. Conclusion We can chose proper bone flap of anterior iliac crest pedicled with vessel to transposing or transplanting according to different objective, and select optimum operation pattern by the complementation of the four vessels ,diameter and rotation range of the vessel pedicels.
