In this paper the pathology of Liver Qi depression in female infertility was studied through theoretical and clinical researches, which is helpful to perfect the TCM theory of reproductive physiology and pathology,and also provide a basic research and a new idea for the syndrome differentiation and treatment of female infertility.In the research background section, the TCM theory of reproductive physiology was cleared up and that of obstruction of ovulation resulted from Liver Qi depression was studied. It was considered that though the female reproduction was conducted mostly by Kidney, it was also closely related to Liver. Connected with the Chong Meridian, the Ren Meridian, the Du Meridian, the Dai Meridian and uterus, Liver is the pivot of the regulation of menstrual cycle and ovulation by its function of regulation the flow of Qi. Liver coorperated with Kidney to regulate the production and the cycle of menstruation and ovulation. Liver Qi depression is prominent in pathological changes of women of childbearing age. It is the basic pathological link in obstruction of ovulation and is one of the pathological bases of female infertility. It was also discussed that the imbalance of reproductive endocrine was the pathological mechanism of Liver Qi depression in female infertility.In the clinical research section, 70 female cases with female infertility were observed, and 19 healthy women as control. With the indexes such as mark of Liver Qi depression, grade of Liver Qi depression, basal body temperature, maturation index(MI) and maturation value(MV) of vaginal exfoliative cell, follicule-stimulating hormone(FSH), luteinizing hormone (LH), estradiol(E2), progesterone(P), testosterone(T), prolaction(PRL) of peripheral blood, the study was carried out. The result showed that:1.The compound syndrome was commonly found in syndromes of female infertility, i.e.syndromes of Liver Qi depression and deficiency of Kidney mixed with other syndromes mostly. Syndrome of Liver Qi depression was the most common syndrome in femaleinfertility. It is concluded that mutual action of multiple factors is the pathological basis of female infertility. Depression of Liver Qi and deficiency of Kidney are the primary pathogensis.2.Liver Qi depression in different extents existed in various syndromes of female infertility, which indicates that Liver Qi depression is the important link in pathological changes of female infertility, and it is one of the pathological bases of female infertility.3.On the 10th day in menstrual cycle (follicular phase), MI and MV of vaginal exfoliative cell in different syndrome groups were normal. On the 20th day in menstrual cycle (luteal phase), MI in the Control Group tended toward the middle zone. But MI in different syndrome Groups slighted moved to right and MV were all higher than that in the control group, which presented the slide type of deficiency of progestogen and indicated the obstruction of ovulation and which might relate to Liver Qi depression.4.In the follicular phase, E2 in different syndrome groups all increased, P in Groups of syndrome of Liver Qi depression and deficiency of Kidney increased, but P/E2 unchanged. In the luteal phase, P and P/E2 in different syndrome groups all discreased. It is concluded that changes of estrogen and progestogen reflect the obstruction of the mutual wax-wane and transformation of Yin and Yang. The obstruction of ovulation resulted from the obstruction of the mutual wax-wane and transformation of Yin and Yang embodies the pathology of Liver Qi depression in female infertility.5.In the follicular phase, PRL of female infertility patients increased, and it was negatively related to E2, P and P/E2 and positively related to LH and LH/FSH. In the luteal phase, PRL was positively related to LH/FSH and the grade of Liver Qi depression. It is inferred that the increase of PRL which affects the function of gonad axis and results in the obstruction of ovulation is the endocrine base of Liver Qi depression.
