BackgroundIn the past 30 years, considerable clinical success has been claimed for functional endoscopic sinus surgery (FESS), which has been an important way to cure chronic sinusitis. It lacks object evidence, especial the evidence about the restoration of mucoaciliary, to assess FESS, although there are considerable literature about the ultrostructure of mucoaciliary restoration, which has been certificated. So the mucoaciliary movement of rhinosinus following FESS is a focus in the Otolaryngology. Otherwise, the methods about establishing chronic sinusitis animal model in literature have some shortcoming, and need a new chronic sinusitis animal model to instead of.Objective1. To compare and discuss some methods about establishing experimental maxillary sinusitis in rabbits and observe their mucoaciliary change.2. Observing the mucoaciliary changed on experimental maxillary sinusitis in rabbits to compare and discuss the effect of different treatment on ostium and disposal sinus, these results offer quantitative evidence of objective improvement following FESS, support the pathophysiological concepts on which the technique is based.Methods1. The first experiment: Sixty six New Zealand white rabbits were divided into seven groups: control group, sham-operation group I, sham- operation group II, bacteria inoculation group, ostia-blocked group, ostia-blocked and bacteria inoculation group, ostium set cotton group. The animals were examined histologically and bacteriologically after 6 weeks.2. The second experiment: Sixty New Zealand white rabbits were to take action. Fifty-four animals to establish chronic sinusitis models by ostium set cotton in maxillary sinus, and the other animals were set in control group. After 6 weeks, chronic sinusitis models set up. According the difference of the treatment on the ostium and disposal after operation, the animal were divided into five groups: sham-operation group(6), ostium opening group(12), ostium opening and lavaging group(12), ostium enlarging group(12). Ostium enlarging and lavaging group. All animals killed after 12 weeks, the movement of mucoacillary will be evaluated by detecting MTR and observing ciliary beat under contrast phase microscope, the ultro-stmcture under light microscope or electronic microscope. And the bacterium in sinus will be detected.Results1. The positive rate of chronic sinusitis in ostia-blocked group are 90%, inoculated staphylococcus group were 100%, ostium set cotton group are 100%, and the other groups are 0%.2. 12 weeks after operation: the positive rate of bacterium in sinus: control group are 0%(0), sham-operation group are 100%(6), Ostium opening group are 100%(12), ostium opening and lavaging group are 67% (8), ostium enlarging group are 42%(5), ostium enlarging and lavaging group are 0%(0). MTR: control group are llmm/min(9.9-12.3mm/mm), sham-operation group are 0.2mm/mm (0-2.7mm/min), Ostium opening group are 2.2mm/min (0-6.2mm/min), ostium opening and lavaging group are 4.5rnm/min(0-9.9mm/min), ostium enlarging group are 5.2mm /min (2.1-10.1mm/min), Ostium enlarging and lavaging group are 7.6mm/min(2.7-n.lmm/min). The beat of ciliary under contrast pase microscope: control group are 100%(6), sham-operation group are 0%(0), ostium opening group are 8%(1), ostium opening and lavaging group are 33% (4), ostium enlarging group are 58%(7), Ostium enlarging and lavaging group are 100%(12). The mocuacillary under light microscope: the normal sinus mucosa in rather thin, covered by a single layer of pseudostratified columnar epithelium and resting on a delicate basement membrane. The epithelium thickness thining, inflammatory cells and Goblet cells decreased, but fibrosis and submucosal glands were not changed. The mocuacillary under electronic microscope: the ciliary are regular, uniform.Conclusion1. Stable chronic sinusitis model can be established by ostium set cotton, it is simplest than other methods.2. Serious inflammation of experiment maxillary sinusitis will be relievd and movement of...
