Aims: To study the expression of hTERT protein in human gastric carcinoma and its precancerous lesions, and explore the relationship between hTERT expression and H.pylori infection. To discover the relationship between the influence of H.pylor infection on hTERT expression and cell apoptosis. To realize the relationship between the expression of hTERT and the expression of bcl-2 and c-myc protein, reveal the relationship between bcl-2 and telomerase activity. Meanwhile, to partly explore the regulation mechanism of telomerase activity and provide the theoretic evidence for illustrating the carcinogenic mechanisms of H.pylori. Methods: using rapid urease test and Warthin-Starry method to detect H.pylori, Using terminal deoxynucleotidy1 transferase-mediated deoxyuridine triphosphate biotin nick end labeling (TUNEL) technique and immunohistochemical method to study the density and distribution of apoptotic cells in situ and the frequencies of hTERT expression, bcl-2 expression and c-myc expression in the 64 specimens. After that, using the flow cyto metryanalysi (FCM) to study the hTERT protein expression in SGC7901 cells transfected antisense bcl-2 oligodeoxy nucleotidsin and the bcl-2 protein expression in SGC7901 cells transfected antisense hTERT gene under the stimulation of low concentration H.pylori incubating filtrate.Results: (1) The respective positive frequency and middle and strong positive frequency of hTERT protein expression in chronic atrophic gastritis, intestinal metaplasia, gastric dysplasia and human gastric carcinoma were 25.00%, 46.67%, 64.29%, 78.95% and 6.25%, 13.33%, 28.58%, 52.63%, which took on a increasing tendency. The positive frequency of hTERT protein expression in human gastric carcinoma was significantly higher than that in gastric dysplasia, intestinal metaplasia and chronic atrophic gastritis (P<0.05), and that in gastric dysplasia was significantly higher than that in intestinal metaplasia and chronic atrophic gastritis (P<0.05), and that in intestinal metaplasia was significantly higher than that in chronic atrophic gastritis(P<0.05). The positive frequency of hTERT in the cases with H.pylori infection was 77.42%, which was significantly higher than that in the cases without H.pylori infection (33.33%, P<0.01). (2) The respective apoptosis index in chronic atrophic gastritis, intestinal metaplasia, gastric dysplasia and human gastric carcinoma were 11.54±2.28, 17.05±2.88, 7.41±1.32 and 5.03±2.23. The apoptosis index in human gastric carcinoma was significantly lower than that in gastric dysplasia, intestinal metaplasia and chronic atrophic gastritis (P<0.05), and that in gastric dysplasia was lower than that in intestinal metaplasia and chronic atrophic gastritis(P<0.05), and that in intestinal metaplasia was significantly higher than that in chronic atrophic gastritis(P<0.05). The apoptosis index in the cases with hTERT protein expression was 8.45±5.30%, which was significantly lower than that in the cases without hTERT protein expression(11.86±4.24%, P<0.05). The apoptosis index in the cases not only with hTERT protein expression but also with H.pylori infection was 9.57±5.01, which was significantly higher than that in the cases with hTERT protein expression but without H.pylori infection(6.01±5.31,P<0.05). (3) The respective positive frequency of bcl-2 and c-myc protein expression in chronic atrophic gastritis, intestinal metaplasia, gastric dysplasia and human gastric carcinoma were 31.25%, 53.33%, 57.14%, 84.21% and 25%, 53.33%, 64.29%, 79.85%, which took on a increased tendency. The positive frequency of bcl-2 and c-myc protein expression in human gastric carcinoma was significantly higher than that in gastric dysplasia, intestinal metaplasia and chronic atrophic gastritis (P<0.05), and that in gastric dysplasia was significantly higher than that in chronic atrophic gastritis (P<0.05), and that in intestinal metaplasia was significantly higher than that in chronic atrophic gastritis (P<0.05).
