Objective: To investigate the appliance value of hydrogen proton magnetic resonance spectroscopy (1H-MRS) in the lateralization diagnosis of epilepsy and in the forepart diagnosis of hippocampi sclerosis. To observe the correspond influence of spectrum caused by the reduce of the times of seizure.Methods: 35 patients with epilepsy, include 21 patients with temporal lobe epilepsy, were collected from March 2002 to February 2004 in internal medicine in our hospital. All of the cases underwent 1H-MRS, MRI and EEG study. Signal from NAA, Cr and Cho were observed. Ratios of NAA/(Cr+Cho), NAA/Cr and NAA/Cho were calculated and analysed. (1) Decide the side of focus based on the result of EEG, symptom and the result of 1H-MRS and contrast the result of EEG and 1H-MRS. (2) All the patients were disparted into two groups: hippocampi sclerosis and not hippocampi sclerosis, based on symptom and MRI. Contrast the radio of the focus and the opposite of the groups. Then compare the radio of focus of the groups to the normal separately. (3) Calculate the difference between the date of focus and the normal, then correlate them with the times of seizure. (4)Discuss the changes on !H-MRS after effective therapy.Result: (1)There was significant statistical differnce in the compare of the radio of the focus and the oppisite side (P<0.001) . The radio of NAA/ (Cr+Cho) in the focus was differ with the normal apparently. So was the compare of the radio of NAA/Cr; (2) In the compare of the group of hippocampi sclerosis and not hippocampi sclerosis, there were statistical differnce between NAA/ (Cr+Cho) and NAA/Cr, but no differnce betweenNAA/Cho. So is the comparison between hippocampi sclerosis and the normal; (3) The difference between the date of focus and the normalhad positive correlation with the times of seizure, but had no statistical differnce; (4) The radio of NAA/ (Cr+Cho) had evidence inproved in both sides after effective therapy.Conclution: (1) MRS provides useful information in the lateralization of patients with temporal lobe epilepsy; combined with EEG, MRS may help increase the rate of diagnosis; (2) MRS ,as a sensitive method in forepart diagnosing hippocampi sclerosis (HS), can improve the detection rate of HS. (3) That the change of the metabolite of hippocampi had positive correlation with the times of seizure need to be approved. (4) The metabolite of hippocampi improved after the decrease of the times of the seizure.
