The Study Of General Practice Model Of Regimental Medical Detachment

Community Health Services have been viewed as an important strategy to reform city' s health service system since 1997,and they have increasingly shown the vigorous vitality and bright perspective. The present model of health services in our army is still the traditional biomedical model that has not adapted to the development of bio-psycho-social medical model. This is especially true for military grass-root health organizations that are responsible for disease prevention, medical treatment, health care and medical support at peace. It has been the key barrier to the development of military health cause.The study on the military grass-root health needs and the health service items of regimental medical detachment was conducted to analyze the general health service conditions of regimental medical detachment and discuss the trend for the health service development of regimental medical detachment, which based on the theory of General Medicine and Community Health Services.Compared with the two populations, aged between 15 and 24 and between 25 and 34,in the national health service investigation in 1998, the military grass-root two-week prevalence rate and incidence rate of chronic diseases were both higher than the same aged population; thediseases were heavier; the disease spectrum was the same that respiratory system diseases and digestive system diseases were the major diseases. Motor-system diseases induced by military training was owned only in the army, respiratory system diseases, digestive system diseases and motor-system diseases was 79.15 % of all diseases.Mental stress in military grass-root officers and soldiers was very common, most of them didn't have psychological consulting because 46.80% of whom didn't know where to go to consult.Regimental medical detachment was responsible for diseases prevention, medical treatment, health education, training-wound prevention and treatment and the training of help oneself save oneself and others etc, which involved in prevention, medical treatment and health care, so in some degree, the task of regimental medical detachment was similar to that of community health service center, but referring to the concrete service, some services were imperfect and some important services were not provided.The grass-root army was similar to community. Compared the responsibility of regimental medical detachment and community health service center, the responsibility of regimental medical detachment ismuch similar to that of community health service center in city which took on the main task of general medicine. But the present services provided by regimental medical detachment were based on the traditional biomedical mode without the guide of community health services and general medicine theory. So regimental medical detachment didn't have some key services including community diagnosis, psychological health and psychological health consulting services and community getting-well service.Conclusions: regimental medical detachment should adopt the general practice model. Proposal: innovate the services provided by regimental medical detachment, provide general medicine education, change the function of medical succeeded education institute, attach importance to general medicine education, establish the evaluation system of military grass-root health services, establish relevant policy and measures.
