As a non-invasive technique for the measurement of blood and oxygen in tissue, the near-infrared spectroscopy (NIRS) has an increasing application in the postoperative monitoring of the plastic surgery. This dissertation expounds the NIRS technology and the theory of steady spatial resolved spectroscopy (SRS), it also describes the application of this technique in the monitoring of free flaps after surgery and tries to resolve the main problems during the clinical practice, such as the design of instruments, the identification of vascular embolism, and so on.This thesis has studied the fibular flap that used in the reconstruction of jaw. And the main contents include: constructing an optical model of the transplanted fibular flaps, and analyze it by Monte Carlo simulating method. Based on this, the present researcher calculates the optimal measuring distance pair when using SRS to examine tissue oxygen index (TOI) in fibular flaps. In the end the thesis discusses how to choose the distance pair when using SRS in the measuring of multilayer tissue.Cooperated with the other colleagues in the lab, the researcher has developed a NIRS oximeter used in the postoperative monitoring of fibular flap, which can fully satisfy the clinical requirements both on safety and stability.Using the NIRS oximeter some clinical experiments have been done in Peking University School of Stomatology, to monitor the healing process of the fibular flaps after surgeries. The researcher also analyzes the changing regularities of TOI in the unsuccessful operations, and gets a tentative conclusion on this changing regularity of TOI in fibular flaps after operations, which is of much help to identify the vascular embolism.In addition, as one of the major technicians, the writer also takes part in the productionalization of NIRS oximeter, which includes the registration, submission of materials, testing and optimizing of this product.
