Objective By using MR cholangiopancreatography (MRCP), to investigate the pathological biliary pancreatic anatomy,determine the differential diagnostic value for patients with periampullary carcinomas.Methods Fifty-four cases of periampullary carcinomas, which were examined by MRCP and confirmed by operation and pathology, were divided into two groups: 28 cases with carcinomas of the head of pancreas and 26 cases with ampullaregion carcinomas (including: 10 cases with distal common bile duct carcinoma, 10 cases with ampullary carcinoma, 6 cases with duodenal papillary carcinoma). Ten patients without bile duct and pancreatic diseases were examined by MRCP as control group. Three groups of patients were examined under same condition and sequence. The original films of MR were amplified to decrease the error. The diameters of common bile duct and pancreatic duct and the angles of the pancreaticobiliary conjunction were monitored by a compasses and ruler in same condition. The correlativity of common bile duct diameters and pancreatic duct diameters were analyzed by statistical software. The differences of common bile duct diameters and pancreatic duct diameters, dilation times of pancreatic duct and common bile duct, pancreatobiliary conjunction angle in each group were analyzed in statistics software.Results Compared to the control group, diameters of common bile duct dilated significantly as the ampullaregion carcinoma groups and carcinoma of the head of pancreas group (F=13.11, q=10.77, 10.36, P<0.01). There were no significant difference in dilation of common bile duct in the two groups(P>0.05).
