Background: The acute pancreatitis is a systemic inflammatory process that come from the pathological assault in abdominal cavity ;Monocyte/macrophages play an important role in inflammation , many proinflammatory cytokines and inflammatory mediators that come from them is the innate mechanism in the inniation, development,concordance, resolution of inflammation;the subpopulation of the macrophages in the abdominal cavity accounts about 8% of the systemic monocyte/macrophages,they are important in the abdominal inflammation and the immunological protection. Aim:The study was designed to clarify the important role of the peritoneal macrophages in the initiantion and progression of acute pancreatitis. Methods:The model of acute pancreatitis was induced in two groups of rats, peritoneal macrophages were depleted in one group rats,and the other group rats were normal,the sham operation group was as control.Peritoneal macrophages were depleted by the intraperitoneal injection of liposome encapsulated disodium dichloromethylene bisphosphonate(liposome-CL2MBP). 1,3,6 and 12 hour after the induction of acute pancreatitis,the pancreas and lung were examined with histological method,the serum amylase/ plasma endotoxin/ ascites and serum cytokine/ascites nitric oxide were examined,blood gas were analysed and the wet/dry ratio of lung were measured;the inflammation grade in pancreas and abdominal cavity,the systemic inflammation,the inflammation and functional status of lung were evaluated through these indexes. The role of peritoneal macrophages in the progression of acute pancreatitis and its systemic complication were evaluated by comparison the three groups,. Result:The depletion of peritoneal macrophages had no significant effect on the pathomorphology modification of pancreas ,so as the serum amylase,but the administration decreased the quantity of ascites and the level of cytokine in the serum and ascites significantly,it also improved the local and systemic inflammation in this model,so as the lung injury and the blood gas analyse parameter,the survival rate of rat increased accordingly. Conclusion: Through modulating the inflammation in peritoneal cavity ,peritoneal macrophages promoted inflammation from the pancreas and the peritoneal cavity to systemic inflammation in acute pancreatitis of rat .The administration of depletion of peritoneal macrophages in rat before induction of acute pancreatitis could improve the pathogenetic condition.It provides the theory basis in some degree for the treatment of pancreatitis by peritoneal lavage.It may be helpful in interpreting the other inflammatory process that come from peritoneal cavity.
