Renal hypertension is the mean reason of secondary hypertension, which accoutered for 10% of hypertension. Renal hypertension is more difficultly in treated being compared with the original hypertension. The indexes of ABPM (Ambulatory Blood Pressure Monitoring) have great correlation with the damage of heart in original hypertension patients and may be used as predictive determinants and study tools for damaged target organs of hypertension. A lot of research have shown the following facts :It is superior to clinical blood pressure in the relativity between the average 24h ambulatory blood pressure, average day blood pressure, average night blood pressure and cardiovascular complications for original hypertension patients, the blood pressure variability have connection with the cardiovascular complications. There is research for the circadian rhythm of blood pressure of renal hypertension, but there is no report about renal hypertension connection with left ventricle hypertrophy with ABPM. So, to study the circadian rhythm of blood pressure between renal hypertension patients and original hypertension patients, circadian rhythm of blood pressure relation to left ventricle hypertrophy for the hypertension patients and urinary albumin excretion rate applicative for the original hypertension patients with ABPM. Methods: Detecting with the circadian rhythm of blood pressure, ECG, and urinary albumin excretion rate of 80 patients of 40 suffering from renal hypertension and 40 of suffering from original hypertension. Result: There is no different in day systolic blood pressure (dSBP) and day diastolic blood pressure (dDBP) between two groups (p<0.05), but there is remarkably different in night systolic blood pressure (nSBP) and night diastolic blood pressure (nDBP) between two groups. The declined rate of night blood pressure: the original hypertension patients night blood pressure declined rate was more than 10%, however the renal hypertension patients night blood pressure declined rate less than 10%. The renal hypertension patients have high connection with left ventricle hypertrophy compared original hypertension patients. The original hypertension patients with urinary albumin rate 30-300mg/24h have higher blood pressure compared with the patients with urinary albumin excretion rate less 30mg/24h. Conclusion: The circadian rhythm of blood pressure of the renal hypertension patients disappeared, but the original hypertension patients still existed. Because the lost of the circadian rhythm of blood pressure, the renal hypertension patients have more higher accruing rate left ventricle hypertrophy. The increasing degrees of blood pressure of the original hypertension patients have direct proportion with urinary albumin excretion rate.
