Objective: Intracerebral hemorrhage (ICH) is one of the most common and frequently occurring diseases. In order to seek more effective therapeutics, people restart to pay more attention to the pathophysiology after ICH. The studies recently indicated that the major mechanism of secondary injury after ICH was the cerebral edema around the hematoma. The animal experiments discoveried that there was an inflammatory response in and around blood clot after ICH. Infiltrating leukocytes and activated platelets take part in the course of inflammation reaction, which suggest that the inflammation reaction may contribute to edema formation and secondary brain injury after ICH. The leukocytes infiltrated and platelets activated as well as the inflammation medium etc. may be the important factors to the cerebral edema. The adhesion of leukocytes and endothelial cells is important to the migration of leukocytes into brain. It may result from up-regulation of adhesion molecules involved in the selectin family, immunoglobulin super family and integrin family. Normally, only few adhesion molecules express on the cell surface, but their expression can be induced by some inflammation courses. CD62p, TSP and ICAM-1 belong to different family, they may play important roles at different stage of the inflammatory response after ICH. CD62p and TSP are significant indicators of activation of platelets. The purpose of the present study was to investigate the expression of CD62p, TSP, ICAM-1 and leukocytes count in the ICH patients and the control group, and make correlation analysis between these factors, the inflammation reaction inside the hematoma and the cerebral edema around the hematoma. The relationship between the adhesion molecules, activated platelets and the inflammation reaction after ICH, and the relationship between the activated platelets, the inflammation reaction and the cerebral edema after ICH were also studied. Methods: There were 33 ICH patients treated with minimally invasive surgery and 21 healthy persons as the control. The venous blood of those people and the hematoma fluid of the ICH patients were collected for detection of CD62p, TSP, and ICAM-1 with Flow Cytometry. The white blood cells were counted with the COULTER STKS automated blood convention analysis apparatus. All the data were analyzed with SAS statistic software. Results: 1 The level of the expression of CD62p and TSP on the platelets surface in the hematoma fluid was higher than that in the venous blood of ICH patients and the control group with significant difference. (CD62p: P<0.01;TSP: P<0.01). 2 The level of the expression of ICAM-1 on the Granulocytic cells and the Lymphocytes surface in the hematoma fluid was higher than that in the venous blood of ICH patients and the control group with significant difference. (Both of them: P<0.01). 3 The leukocytes count in the hematoma fluid was higher than that in the venous blood of ICH patients and the control group with significant difference. (P<0.01). 4 There was a significant positive correlation between the level of the infiltrating leukocytes inside the hematoma and the volume of the brain edema after ICH. (r=0.9516, P=0.0001). 5 There was a significant positive correlation between the level of the expression of CD62p and TSP on the platelets surface and the infiltrating leukocytes inside the hematoma after ICH. (CD62p: r=0.5374, P=0.0013; TSP: r=0.5468, P=0.0010). And there was a positive correlation between the level of the expression of CD62p and TSP on the platelets surface in the venous blood of ICH patients and the infiltrating leukocytes inside the hematoma after ICH. (CD62p: r=0.4401, P=0.0104; TSP: r=0.4288, P=0.0128). 6 There was a positive correlation between the level of the expression of ICAM-1 on the Granulocytic cells and the Lymphocytes surface and the infiltrating leukocytes inside the hematoma after ICH.
