Objective: Hyperlipemia is one of the clinical symptoms of nephrotic syndrome (NS), some scholar reported that there are more than seventy percent patients have this symptom. At present it is regarded that continuous hyperlipemia can accelerated mesangial cell proliferation and extracellular matrix accumulation, and at last lead to glomerulosclerosis and interstitial fibrosis. The character of lipid metabolism in nephrotic syndrome is that almost various kinds of lipoprotein increase, and among them the total cholesterol is increased in the early stage, the most significance is low density lipoprotein. Liver store most of the low density lipoprotein-receptor (LDL-R) and is the main organ to degradation LDL. In the NS, excessive lipid deposited in mesangial area, then the endothelial cell, mesangial cell and infiltrated macrophage can release oxygen-derived free radidicals to modificate LDL, and then formed into Oxidative low density lipoprotein (OX-LDL). OX-LDL and lipid peroxidation production (such as malondialdehyde) can accelerate the glomerulosclerosis and interstitial fibrosis. Nephrotic syndrome belongs to"edema"in traditional medicine. My tutor regards that its mechanism is owing to righteousness abcence, qi stagnat, blood stasis and water stopping. So we use the method of promote qi circulation to induce diuresis, supplementing qi and activating the blood circulation, disperse blood stasis and dredge collateral. In the direction of this method and the recent rearch achievement of herb pharmacolog, we make a compound prescription of Chinese herbs-ChaiLingGuiQi decoction, which is composed by ChaiLing decoction and BuYangHuanWu decoction. The model of Adriamycin-induced nephropathy rats is one of the NS models in experiment. We intervene with the Adriamycin-induced nephropathy rats with ChaiLingGuiQi decoction in this topic, observing the influence of this prescription to proteinuria, plasma protein, lipid metabolism, lipid peroxidation, renal function, renal pathology of the model. We adopt reverse transcription polymerase chain reaction and immunohistochemistry methods, observing the expression of liver LDL-R mRNA, laminin and fibronectin of renal tissue. At the same time, we select Simvastatin as control to evaluate the effect of ChaiLingGuiQi decoction, and to explore its possible mechanism to improve lipid metabolism, so as to provid scientific experimental foundation for its clinical application. Method: Thirty-six male Sprague-Dawley rats were selected to our experiment. After one week, they were put into metabolism cage to obtain urine, and urine protein and red blood cells were detected. The result was negative. Then they wererandomly divided into four groups: Normal, Model, Simvastatin and ChailingGuiQi group (n=6,in normal group; n=10,in each other three groups). Each animal adopted tail intravenous injection of Adriamycin according to 5mg.kg-1 except the Normal group. Drugs were administered from seventh day after duplicating model but the saline were used in Normal and Model groups orally. At the second, third, forth, fifth, sixth and seventh weekend,the rats were put into metabolism cage to obtain urine for the 24-hour's urine protein. Animals were sacrificed after 7 weeks, proceeded by the routine and biochemistry analysis. Some renal tissue was obtained for SOD and MDA, some for the pathomorphology, and by the means of immunohistochemistry methods to observe the expression of laminin and fibronectin, some liver was obtained for RT-PCR of LDL-R mRNA. Result:(1) 24h urinary protein quantitative analysis showed that: At the second weekend, each group had not showed significant proteinuria. At the third, forth, fifth, sixth and seventh weekend, the Model group was significantly higher than Normal group (P<0.01).
