Objective: To investigate the effect of ulinastatin(UTI) on the perioperative changes of IL-6, TNF- α , IL-10 , IL-4 and the expression of their mRNA in response to surgical correction of adolescent idiopathic scoliosis(AIS) . Methods: Twenty patients were randomly divided into two groups. In UTI group (group U, n=10), patients received intravenous infusion of UTI 10 000/kg just before operation, while patients in control group (group C, n=10) received same amount of saline instead of UTI. Venous blood samples were collected immediately before induction of anaesthesia (T1), 10 min after induction (T2), 1 h after UTI administration (T3), 30 min after extubation (T4) and 24 h postoperatively (T5). Result: At T3, T4 and T5, plasma IL-6, TNF-α and their mRNA increased significantly in group C(P<0.05 or 0.01) ; while those cytokines and their expression of mRNA showed no difference in group U (P>0.05). At T4 and T5, plasma IL-10 and its mRNA indicated a significantly increase compared with T1 (P<0.05 or P<0.01) in group U and were higher than those of group C(P<0.01). The plasma IL-4 and IL-4 mRNA did not change significantly in both groups. Conclusion: UTI could inhibit the release of IL-6, TNF- α and the expression their mRNA may promote expression of IL-10 and IL-10 mRNA.
