Objective: 1) To approach the effects of stabilization splint on TNF- a ? IL-1 β, HA concentration in synovial fluid from TMJs in patients with TMJ disk displacement.2) To analyse the relationship of TNF- a ? IL-l β, HA concentration with symptom of pain. 3)To evaluate the splint therapy on diak displacement patients.Materials and Methods: Synovial fluid aspirates were obtained from 38 TMJs in 29 patients with disk displacement as a splint therapy group undergone 3 months of stabilization splint therapy.8 TMJs in 8 patients with the same diagnosis were a control group in which no management was undergone. Synovial fluid aspirates was obtained in both the moment before and after 3 months.We analysed the TNF- α , IL-1 β, HA concentration in synovial fluid by radioimmunoassay, evaluated the TMJ functional condition and pain intensity by CMI and VAS,and compared the difference of TNF- α, IL-1β, HA before and after splint therapy with difference of VAS by correlation analysis.After spliting the splint therapy group by diagnose into reducing disk displacement group and non-reducing disk displacement group, we discussed the effect of splint therapy on TNF- α , IL-1 β, HA concentration in synovial fluid of different disk displacement subtypes.Results: TNF- α , IL-1 β concentration decreased significantly after splint therapy, while HA concentration did not change significantly.TNF- α , IL-1 β, HA concentration in control group did not change significantly.Showing the TMJ functional condition was improved and release of pain, CMI and VAS decreased after splint therapy. CMI and VAS in control group did not change significantly. TNF- α , IL-1 β,concentration decreased significantly in reducing disk displacement group after splint therapy, while TNF- α , IL-1 β, concentration did not change significantly in non-reducing disk displacement group.
