| Objective To investigate the clinical effect of vitreoretinal surgery (VR) for complicated vitreoretinopathy .Methods To analyze 312 eyes of 306 cases(Retinal attachment 282 eyes)of complicated vitreoretinopathy who treated with VR. combined with scleral buckling/phacoemulsification/ closing macular hole with auto-serum /silicone oil tamponade. Results On discharge ,Retinal reattachment occurred in 259 eyes(91.8%)postoperatively. 282 of 312 eyes followed a median of 12.7months(3~ 38months) .Retina was completed attached in 201 of 257 eyes (78.2%) postoperatively. Postoperative visual acuity improved in 208 eyes(73.8%),and 0.05 or better in 181 eyes (64.2% ) .VR could treat effectively foreign body of optic papilla and subretinal foreign body that the operation was all successful 100%). The macular hole were all closed with VR combined with auto-serum postoperatively in long time.The major intraoperative complication was iatrogenic retinal tear ( 5.8 % ) ,and the major postoperative complications of VR were cataract(25.9%),retinal detachment(21.8%), secondary glaucoma (9.9% ) and keratopathy(9.2%) respectively. Conclusions VR is an effective procedure for complicated vitreoretinopathy. VR is effective for treating foreign body of optic papilla and subretinal foreign body, VR combined with auto-serum could treat effectively retinal detachment with macular hole and VR combined with phacoemulsification for eyes with coexisting cataract and retinal detachment was safe and effective in selected patients. |
