The Effect Of Hydroxyapatite Orbital Implant For Plastic Surgery Of Ophthalmology

[ Purpose ] To evaluate the effect of hydroxyapatite orbital implant for plastic surgery of eye.[Method] We had undergone 3 kinds of procedures for 42 hydroxyapatite orbital implant cases during the past 2 years.24 cases in group I implanted HA enwrapped self-sclera shell at the first stage.6 cases in group II implanted HA covered double-deck sclera cap at the first stage.12 cases in group III implanted HA enwrapped other-sclera shell at the second stage.3 cases with narrowing conjunctival sac received conjunctival sac reconstruction at the time of HA implantion.[ Result] Follow-up periods were ranged from 3 months to 24 months. Eyes pain, balbar conjunctival congestion and tumefaction occurred early in most cases. The symptoms disappeared after allopathy. After artificial eye slice fixed,most cases have gotten plump eyeball and obtained certain eyeball activity(excellent:66.7%;good:21.4%;poor:11.9%). 2 cases(implanted at the second stage ) among them took place balbar conjunctiva split andimplant exposure. Both of them recovered after the mucous membrane of body lip implantation. 2 cases took place conjunctival tumefaction and the conjunctiva extruded out of the palpebral fissure.Both of them recovered after the suture of eyelid. Infection happened in one case,recovering after anti- infection treatment.[ Conclusion] The hydroxyapatite is a kind of artificial synthesis biomaterial with good biological compatibility.It'sporous,nonpoisonous and light.It won't be repelled or absorbed. The stimulation and oppression to the organization of the eye ball is weak. All these advantages are propitious to the fibro-vascular tissue ingrowth into the implants. And once the vascularization is completed,it will possess the host's immunity,and the happening rate of infection , repelletion and shift will decline. So, hydroxyapatite orbital implant is most perfect in the present. We can receive the satisfactory result to apply in clinic. It is more satisfactorily planting into in the first stage than the second stage .The patients with narrowing conjunctival sac received conjunctival sac reconstruction at the time of HA implantion.The complication of the HA implantation include balbar conjunctiva split and implant exposure, conjunctival tumefaction and infection et al.
