Clinical Study In Cirrhotic Patients With Portal Hypertension

Objective: 1) To evaluate the correlation between variceal bleeding and other clinical indexes in patients with cirrhosis. 2) To evaluate the correlation between clinical indexes and esophagus varices in patients with cirrhosis. 3) To study the relationship between plasma nitric oxide and endothelin-1 levels and esophagus varices , ascites in cirrhotic patients with portal hypertension.Methods: 1) Bleeding and other clinical indexes were classified and were graded in 76 patients with cirrhosis, then Spearman rank correlation and Logistic regression of ordinal categorical dependent variable were finished. 2) endoscopic variceal appearances and other clinical indexes were classified and were graded in 76 patients with cirrhosis, then Spearman rank correlation and Logistic regression of ordinal categorical dependent variable were finished. 3 ) Endoscopic variceal appearances, portal vein diameter, serum albumin, ascites, plasma nitric oxide and endothelin-1 levels in patients with cirrhosis were measured, then Spearman rank correlation were finished.Results: 1) Multiple Logistic regression analysis revealed the followingvariables as having statistical significance: endoscopic variceal total score(3.051108,P<0.01) , prothrombin activity (%) (-1.090042,P<0.05) ,Child-Pugh total score ( 0.306172,P<0.15). 2 ) Multiple Logistic regressionanalysis revealed the following variables as having statistical significance: platelet count (PLT) (-1.455995, P<0.01) , the inside diameter of portal vein (1.093647, PO.01), prothrombin time (PT) (0.674698, P<0.05). 3) There is a positive correlation between endoscopic variceal appearances, portal vein diameter and plasma nitric oxide levels as well as plasma endothelin-1 levels. There is a negative correlation between serum albumin and plasma nitric oxide levels as well as plasma endothelin-1 levels . There is a positive correlation between ascites and plasma endothelin-1 levels.Conclusion: 1) The order of the influence to variceal bleeding was: endoscopic variceal total score, prothrombin activity(%),Child-Pugh total score. 2) The order of the correlation to esophagus varices was: platelet count (PLT), the inside diameter of portal vein , prothrombin time (PT). There is a negative correlation between esophagus varices and platelet count. 3) Plasma nitric oxide and endothelin-1 levels were involved in the pathogenesis of portal hypertension.
