PURPOSE: To discuss the value of myocardial perfusion MRI with functional analysis about evaluating the diagnosis and therapeutic effection on the acute myocardial infarction. And to assess the viability of myocardium. MATERIAL AND METHOD: Turboflash ,cine-MR were performed on 18 AMI patients and 8 volunteers to study fiest-pass myocardial perfusion,delay perfusion and function. 8/18 receiveed thrombolysis therapy and were revascularised successfully. To determine the parameters of thrombolysis and non-thrombolysis quantitatively by drawing the time-signal intensity curves and then analysis the variances between them. One-way ANOVO was analysed on prognosing the relationship between myocardium viability and the heart functional parameter of the thrombolysis, non-thrombolysis and volunteers.RESULT: Slight hyperenhancement and hyperenhancement appeared on first-pass and delay-phase in revascularizative patients' infarction.The time of upslope prelonger.These was no significant diference between the upslope of the time-signal intensity curves and the contrast enhancement ratio of these patients and mormal.Hypoenhancement on first-pass and hyperenhancement on delay phase appeared in patients who didn't receive thrombolysis therapy.There were significant difference between the upslope ,the time ofupslope,peak time of the time-signal intensity curves and contrast enhancement ratio of those patients and normal.The peak signal intensity of thromboysis and non-thrombolysis infarction were 85±13%> 53 + 7.1% of that of normal respectively.The ejecting fracture and peak filling rate decreased in patients with myocardial infartion.lt is significantly different with the volunteers.The end diastolic wall thichness and the wall thichen percent decreased more or less.CONCLUSION: First-pass and delay phase myocardial perfusion MRI can determine the position of infarction correctly and reflect the blood supply of the myocardium.Global and regional heart function changes can be comprehended by heart functional analysis. Combine myocardial first-pass, delay phase perfusion and functional analysis can prognosis the viability of myocardium.
