Objectives: to evaluate the value of Free PSA/ total PSA(F/ T)ratio and PSAD in the diagnosis of prostate cancer (PCa) and to distinguish PCa from BPH. Methods:The blood serum PSA, F/T ratio and PSAD of in during 2002 and 2004 of our hospital were studied. 32 healthy control subjects were compared with 39 PCa patients and 111 BPH patients from our hospital during 2002 and 2004. data of blood serum PSA, F/T ratio and PSAD were studied by fluorescence immunoassay, statistics were analysized by mean±standard deviation ANOVA and chi square test. Results: 1, The ratios of FPSA/ TPSA(X|-±s)were 0.35±0.08,0.28±0.07 and 0.13±0.07 in the group of healthy controls,BPH patient and PCa patient srespectively.when 0.16 cut-off value was used , the sensitivity was 66.67% and 24.32% in PCa and BPH respectively, the specificity was 71.87% and 75.0% in two groups, and the differential diagnosis efficiency was 84.50% and 69.01%.the information we gained of F/T rataio whether is higher than the tPSA in diagnosis efficiency. 2,PSAD is correlated to tPSA (P<0.05), PSAD can eliminate the effect because of the volume of prostate.If using 0.15 as the cut-off value,PSAD can raise the specificity and sensitivity,when the PSA between 4.0-10.0ug/L,The PSAD value of PCA group predominancely higher than BPH group(P<0.05).then,in the interval, the isolating rate(84.00%)of PSAD relate to PCA exceeded the tPSA.thus, when tPSA is light and PSAD exceeded 0.15. Conclusions: The ratio of FPSA/ TPSA and PSAD were more valuable than tPSA in diagnosing PCa and in differential diagnosing PCa and BPH, especiallyin the patients with tPSA in 4.0-10.0ug/L.As a result,we can improve the tPSA screening accuracy in combination with the F/T ratio and PSAD,and raised the diagnosis efficiency of tPSA. Combine F/T ratio and PSAD is more accurate in tPSA screening and can serve as a effective method in tPSAdiagnosis.
