Objective To observe the efficacy and explore methodology of dynamical optimizing the system of implantable cardioverter defibrillator in improving quality of life of patients.Methods 41 patients, male 26 and female 15, mean age of 48.7 years were implanted transvenous ICD system each, including 3 biventricular pacing/defibrillating, 4 double chamble and 34 single chamble ICD. Inquiring the data storing in ICD, and selecting some makers, such as ECG, Holter monitoring electrocardiogram, ultrasonic cardiogram, X-ray, 6 minutes walking test, sores of questionnaires of quality of life and so on , were performed for evaluating the efficacy of ICD to treat ventricular tachycardia/ventricular fibrillation(VT/VF), mortality, complications, the sores of quality of life and so on.Results In the average follow-up period of 48.2 months, 34 out of 41 patients(82.9%) received ICD therapy. The data stored in ICD showed that there were total 916 VT/VF episodes detected by the devices. 910 of them(99.3%) were terminated effectively by antitachycardia pacing(ATP), cardiovertion(CV) and/or defibrillation(DF). 6 times invalid turn on shock in 1 patient, occupy 1.7%. 5 of 41 occurrence complication. 1 slices infection, 1 ventricle electrode took off, 7 times sinus tachycardia were soon identified for VT by the mistake and issued ATP and CV in 3 patients. 7 of 41 died. 4 died of advanced heart failure, 1 committed suicide, 1died after stopping himself to reach amiodarone drugs taken orally to use up to the end battery in VT/VF, and another one died of VT/VF because of economic reason, ICD did not replace after using up the battery. The scores of SCL-90 and SF-36 were higher than before optimized ICD system(P < 0. 05).Conclusion Optimizing ICD system can ensure therapeutic efficacy and improve significantly survival probability and quality of life for patients with ICD implanted. It is important to improve the efficacy of follow-up according to different patients and time and pay attention to the principle of saving energy.
