At present, Systemic Inflammatory Response Syndrome (SIRS) is believed that the etiological factors of infection and non-infection effect on organ ,which lead to whole-body inflammatory reaction clinical syndrome. It is named by ACCP,SCCM in 1991 that Hacteremia,Sepsis and Septic shock should be substitued by SIRS. when SIRS develop further, organism will lead to multiple organ dysfunction syndrome (MODS) and become one of important death cause of clinic serious illness. For this reason, predicting SIRS in earlier period and intervening it actively will be the key of prevention and cure MODS. In essence, SIRS is the procedure of inflammatory medium releasing excessively. cytokines,inflammatory cells are interacted on constitution and organ, which will lead to functional abnormal. The processes of SIRS are devided into three phases: the first phase is local response phase, in which produce plenty of cytokine; evoke inflammatory reaction; accelerate wound plerosis; activate reticuloendothelial system; In the second phase, a sprinkle of cytokine enter circulation to reinforce local response, macrophage and blood platelet aggregating and cytokine reproducting; the third phase is the phase of SIRS, that is stable state is broken. If the state of an illness continue aggravating , SIRS is destined to develop to MODS. In recent years, that severe infection and non-infected excitation are researched on cell and molecul level, which have made clear that the SIRS is excessive cytokine and inflammatory media leading to inflammatory reaction out of control and immune disorder. The occurrence and development of SIRS is decided on exciting intensity and response of organism. IL-10 is one of major anti-inflammatory cytokine in the key position of cytokine network ,and play an important role in anti-inflammatory and immunoloregulation. Just like stimulating factor of cytokine, IL-10 can protect constitution to avoid over-destructing, the other hand, IL-10 can reduce immunological function and weaken the ability of eradicating microorganism, even to "immunal paralysis state", and the risk of the twice infection is increased notably or initial infection aggravate. Resently, most scholars think that SIRS may be a kind of polygene disease. The polymorphism of cytokine may influence on production,response intensity and play important role in some inflammatary disease. Genotype of cytokine may be provid theoretical basis of genetics diagnosis,prevention and cure,immunity of some inflammationary disease. The purpose of this study is to ques the predisposing gene of SIRS by analysising the polymorphism of the IL-10 gene of newborn in SIRS, so that we can intervene earlier in the initial period and reduce the incidence of SIRS of newborn. The method of experiment is as following: Fifty-four newbornswith SIRS serve as observing group, fifty normal newborns serve as control group. From every newborn,we draw one millilitre fresh blood with EDTA and without EDTA, respectively. then we measure IL-10 level of serum and draw DNA of lencocyte from blood by the method with saturation Phenol and Chloroform, then we amplificate the IL-10 1082 site gene, using the DNA polymerase chain reaction technique employing the primer gene designed. The amplification products were employed electrophoresis in 2% sepharose and use restriction fragment length polymorphism to investigate the polymorphism of the IL-10 gene. at last, The final products were employed electrophoresis in 4% sepharose and analysis by GIS system. The result of the experiment is as following: Of the observing group, the distribution frequency of the IL-10 genetype GG, GA, and AA is 61.1%, 33.7%, and 5.6% respectively, different from the distribution frequency of the control group (32.5%, 45%, and 22.5%, respectively) analysised by statistacal method. We find the significant association between the IL-10 genotype and SIRS of the newborn. Comparing GG genotype frequency of IL-10 in two groups, we find that GG genotype distribution frequency is 61.1%, higer than control group, the difference has statistacal significance. The distribution frequency of the G allele in the observing group is 77.8%, higher than that of the control group (55%). This show that G allele perhaps is the susceptibility gene of SIRS.
