Objective: To analyze the clinical significance of the swollen lymph node in radical resection; To research for the rule of lymph stretch flow for gastric cancer; To study pathology situation of sentinel lymph node whether or not to delegate pathology situation of its region; The sentinel node navigation surgery can whether or not apply it to surgery therapy of gastric cancer.Methods: Part A: To review case history of original gastric cancer in 463 patients and to select 196 patients which has integrity data of swollen lymph node and total lymph node number and the lymph node metastasis number, and analysing the relationship between character of swollen lymph node and the lymph node metastasis rate. Part B: First of all, to select gastric cancer patient which accord with the condition of sentinel lymph node biopsy. Secondly, the sentinel lymph node biopsy was put into execution. Thirdly, marking SLN. Fourthly, the radical resection was performed. Fifthly, the routine way of HE dyed was put into pathology examination for the SLN, n-SLN and cancer-organization.Results: Part A: a total of 3305 lymph node were obtained from the 196 specimens; There were a total of 1037 swollen lymph node ; There were a total of 423 lymph node metastasis and the lymph node metastasis rate is 12.80% (423/3305) . The lymph node metastasis rate was increased with increasing the swollen lymph node length diameter, pliability and representing nodulus. Part B: We had successfully gained SLN in 66 patients from 76 total gastric cancers and successful rate was 86.84% (66/76) . We gained 81 sorts SLN in successful SLNB patients and 45 sorts (35 patients ) was pathology positive, 36 sorts (31 patients) is pathology negativity. 35 patients was real pathology positive in this grasp and for forecasting pathology status in this area, the SLNB accuracy was 80.30% (53/66), sensitivity was 72.90% (35/48).
