| Objective:Spontaneous intracerebral hemorrhage has remained a serious disease nowadays. It causes higher mortality, disability and makes a big burden to families as well as society. This study was designed to identify modifiable risk factors for spontaneous intracerebral hemorrhage of Chinese Macau Citizen. To explore the risk factors of 207 cases of spontaneous intracerebral hemorrhage under retrospective analysis compared with those patients who admitted at same period without SICH, in order to give theory for prophylactic purpose and give the health education to Macau citizen about these risk factors in order to reduce the incidence of the spontaneous cerebral hemorrhage of Chinese Macau Citizen. Methods:We studied all patients with SICH admitted to Centre hospital of Macau between May 1st ,2001 to March 30th ,2004. The patients were suffered from spontaneous intracerebral hemorrhage admitted from Emergency Department and transferred to Neurological, Neurosurgical ward and Intensive Care UnitTotal 207 consecutive patients with spontaneous intracerebral hemorrhage age ranged from 17 to 101 years, average age 66±16.9years were compared with those of non-SICH hospitalized control patients (190 men and 187 women) who were admitted at same period in similar clinical situation. According to the age distribution, accompanied disease, risk factors and etc, studied and analyzed the risk factors by used SPSS11.0.software. Risk factors use logistic regression analysis, categorical variables were compared by x2 test, continuous variables expressed as t-test. Results:In 207cases of Chinese Macau Citizen, the ratio of SICH in M:F=1.3:1. Age distribution <45 years old(12.1%), 45-75(53.1%) and age above 76 years(34.8%);Incidence of SICH progressive increase with age, the peak incidence in age 70-79 years old.Case subjects of SICH compared with non-SICH control subjects no difference in sex. The incidence of SICH prominent in age 45-76, compared with control subjects with significant difference (P<0.05). Among 207 SICH case subjects, significant different between the cases and control subjects including: hypertension (70% among case subjects verus 34.2% control subjects pO.OOl), diabetic mellitus (25.1 vs 15.1 pO.01), alcoholic drinking (26.1 vs 16.7 pO.OOl), overweight (65.2 vs 19.9 p<0.001)(Table 2). Among 207 SICH case subjects, 35 case subjects were cured, cure rate 16.9%. Moreover, among 377 control subjects of non-SICH, 304 were cured, cure rate 80.6% (16.9 vs 80.6 pO.OOl) (Table 2).Among the young and middle age of SICH patients compared with the old age, no significant difference in sex (p>0.05). Among 131 old age SICH patients, 100 case subjects suffered from hypertension (76.3%). However among 76 case subjects of young and middle age SICH only 57% suffered from hypertension (57 vs 76.3 p<0.05), habit of drinking(16.8 vs 42.1 pO.OOl). But diabetic mellitus, hyperlipidemia, cardiac disease, smoking habit, anticoagulation treatment and overweight no significant difference between both groups.Among 76 SICH case subjects of young and middle age, 17 case subjects were cured, cure rate 22.4%. However, among 131 case subjects of old age SICH, 18 were cured, cure rate 13.0% (22.4 vs 13 pO.OOl).Multivariate logistic regression and correlation analysis revealed: correlation coefficient for hypertension & SICH is 1.54, diabetic mellitus & SICH is 0.523, current smoking & SICH is 0.539 and overweight & SICH is 2.04.Conclusion:SICH common exist in age above 50 years old local citizen, in old age group they usually accompanied with hypertension. However among young and middle age accompanied with relative higher rate of alcoholic drinking. The prognosis of middle and old age were better than old age. We found a high frequency of modifiable risk factors of Chinese Macau Citizen including hypertension, diabetic mellitus, smoking and overweight. These observations suggest that effects directed toward modification of risk factors profile, particularly active treatment of hypertension, prevent and treat diabetic mellitus, quit smoking and weight reducing could potentially reduce the incidence of SICH of Chinese Macau Citizen. |
