Objective: To study the clinical features, environmental risk factors, gene polymorphisms of nonalcoholic fatty liver disease (NAFLD) and interaction between gene and environment factors for early diagnosis and prevention. Methods: A retrospective case-control study was used to explore the clinical features and related risk factors of NAFLD with logistic regression analysis. Genetic polymorphisms of apolipoprotein B, Câ…¢, E and cytochrome P450â…¡E1 were determined by the technique of DNA chips. Results: ( 1) Levels of BMI, WHR, blood pressure, GLU, UA, TG, TC, LDL-C, ApoB, blood viscosity (200S-1, 30S-1), high shear reduction viscosity, red blood cell aggregation index and red blood cell rigidity index of NAFLD group were higher than these of control group. (2) The incidence of obesity, diabetes mellitus, heart and brain blood vessel disease, hypertriglyceridemia and hypertension in NAFLD group were higher than these of control group. Of 324 NAFLD subjects, metabolic syndrome was found in 81 (25%), which was significantly higher than that of control group(5.6%). ( 3 ) WHR, male and hyperlipidemia were positively correlated to the occurrence of NAFLD, while some daily action was negatively correlated to the occurance of NAFLD. (4) Significant differences were found in distribution of frequencies of polymorphisms in the ApoB11039G/A and ApoB7673C/T between NAFLD group and control group. (5) In the ApoB11039G/Apolymorphism, LDL-C and HDL-C levels of the GA genotype were significantly higher than these of the GG genotype. (6 ) In ApoB7673C/T polymorphism, CT and TT genotype had shown a significant gene-environment interaction with both WHR and hyperlipaemia in the occurance of NAFLD. Conclusions: (1) NAFLD may be considered an additional feature of the metabolic syndrome (MS), it is more important to control the heart and brain blood vessel disease which is related to MS. (2) The occurance of NAFLD is related to life styles, to prevent NAFLD and its syndromes, controlling obesity, changing bad life styles and periodically physical examination were mostly important. ( 3 ) ApoB11039G/A and 7673C/T polymorphism are associated with the occurrence of NAFLD. The study showed significant interaction between inherited factors and environmental factors in NAFLD, therefor, for early diagnosis and prevention, the polymorphisms of ApoB 11039G/A and7673C/T should be determinate to screen susceptive persons, and then direct these persons to controll obesity, change bad life styles and ensure periodically physical examination.
