Objective: The present study was to investigate the role of tumor necrosis factor-alpha (TNF-α) and intercellular adhesion molecule-1 (ICAM-1) in the pathogenesis of viral myocarditis and study the protective effect of anti-TNF-alpha monoclonal antibody (anti-TNF-αmAb) on myocardial injury in Mice with myocarditis induced by coxsackievirus B3 (CVB3).Methods: One hundred and thirty-two male Balb/c mice of 4-6 weeks were randomly divided into three group :the normal control group, the CVB3 infection group and CVB3 infection with TNF-a antibody treatment group (the treatment group). The expression levels of ICAM-1 and TNF-α the ultrastructure of heart, the pathologic changes and the survival rate were studied in 7 and 14 days after infection, the relationship between the expression levels and myocardial damages were studied statistically by semi-quantitative analysis.Results: As compared to infection group, the expression of ICAM-1 and TNF-a reduced in treatment group (the 7th day:14.71 ± 1.94vsl8.80±2.62, P<0.0/andl6.70± 2.90vs20.63 ± 1.95, P<0.05; the 14th day: 17.49 ±2.58vs23.51 ±3.50, P<0.01and 19.29 ±3.21vs24.64±2.97,P<0.01); The changes of ultrastructure and pathology reduced, the survival rate increased in the treatment group than that in infection group in 7 and 14 days after infection; The 14-day survival rate in infection group was 62.5%, that in the treatment group increased 87.5% (P<0.05); Using the correlation analysis, a positive correlation between the expression of ICAM-1 and TNF-a (r=0.706, P<0.05), the expression of ICAM-1, TNF-α and the changes of myocardial histopathologic score (r=0.737, P<0.05; r=0.693, P<0.05)in infection group.Conclusion: A positive correlation between the expression of ICAM-1 TNF-α and the changes of myocardial histopathologic score, ICAM-1 and TNF-α should be important roles in the pathogenesis of viral myocarditis, anti-TNF-alpha monoclonal antibody can reduce expression, reduce the changes of ultrastructure and pathology, improve the survive rate and protect myocardial tissue.
