Cardiovascular disease is seriously threatening to human's health, the study hasshowed that its development is close related to the disturbance of vascularendothelium. The therapeutic mode dominated by western medication has manydeficiencies, so the new therapeutic methods and strategies with TCM are necessaryto be developed. Cardiotonic pill, a preparation composed of Danshen, Sanqi andBingpian, has been clincally used for the treatment of cardiovascular disease, but itsmechanism, the relation of efficency and active ingredients are not clear.The microcirculation and survey system with continuousness, visualization andvarious indices was used to observe effect of cardiotonic pill on ameliorating ratmesentery microvascular endothelium dysfunction due to ischemia-reperfusion. Thedifferent actions and mechanisms of preparation, single herb and active ingredientswere tested. The results showed that cardiotonic pill can scavenges peroxide, inhibitsleukocyte adhesion to vascular endotheliocytes, but also inhibits albumin leakageand mastocyte degranulation, Danshen and Danshensu's actions are relate toalleviating peroxide, inhibit leukocyte adhesion and albumin leakage. Notoginsengand notoginsenoside can inhibit leukocyte adhesion and reduce mast celldegranulation. The actions of single herb and active ingredients are different fromthe protection of microvascular injury. Cardiotonic pill has the comprehensiveprotective effects on the damaged microvascular endothelium relative to single herband active ingredients.To explore the protective mechanism of cardiotonic pill at cellular andmolecular levels, vascular enthdolial cell (VEC) and vascular smooth muscle cell(VSMC) were used in vitro culture. The results showed that cardiotonic pillsignificantly attenuates tumor necrotic factor (TNF) α-induced expression ofadhesion molecule ICAM-1 and VCAM-1, inhibites platelet-derived growth factor(PDGF) BB-induced DNA synthesis and VSMC proliferation.Cardiotonic pill has the special advantage for the protection of vascularentothelium by inhibiting leukocyte adhesion and scavenging peroxide. At thecellular and molecular levels, cardiotonic pill can attenuate the expression ofadhesion molecule and inhibite VSMC proliferation. The results are helpful toprovide the new method for the treatment of cardiovascular disease and toaccelerate the procedure of internationalization of cardiotonic pill.
