Objective1. To investigate the prevalence of depressive disorders in Liaoning province.2. To describe the association between prevalence of depressive disorders and demographic variables, such as sex, urbanization, age, marriage, education and income.3. To calculate the burden of disease of depressive disorders in Liaoning province using DALYs.MethodStratified multi — stage cluster randomization was used to identify 3 cities (Shenyang, Anshan, Fuxin ) and 3 countries ( Dawa, Qingyuan, Zangwu ) and 15516 subjects of 18 -64 years old. The respondents were visited by lay interviewers at their home and conducted a face - to - face interview. Composite International Diagnostic Interview was as the screening and diagnosis tool. The diagnoses of derpessive disorders were made according the 10th of International Classification of Disease criteria. The standardized rate was calculated as the world population. The statistical analysis was used as SAS software (Version 6. 12) . To describe the relationship between the prevalence of depressive disorders and risk factors using ORs of logistic regression analysis, the significance wasused by Wardx2.DALYs for depressive disorders are the sum of the years of life lost because of premature mortality in the population and the years lived with disability. DALYs = YLLs + YLDs. Because depressive disorders can not cause death, so DALYs = YLDs. The forluma for calculating the YLDs is:/ -pxe"r(x"a)dxx =a= NDCera/(p+r)Ce-(p+r)(L + a)[ -(p + r)(L + a) -1] -e'(p+r)a[ -(p+r)a-l] The incidence of depressive disorders is rarely measured. To derive the incidence , we calculated it from one - year prevalence. The prevalence of depressive disorders of 18 -64 years old was derived from the survey and that of aged 17 years or younger and aged 65 years or older was derived from the estimate of epidemiological expert. The incidence of major depression was calculated from the formula of P = ID (P is the Prevalence, I is the Incidence, D is the Duration ). We made extensive use of DisMod software package developed by Harvard University to examine the estimate of incidence and duration of dysthymia from the prevalence, remission and relative risk. The disability weight was calculated as product of the each weight and the distribution of depressive disorders.resultA total of 13358(86. 1% of the eligible subjects) agreed to participate in the study and were successfully interviewed. The prevalence of depressive disorders in Liaoning province was 2. 24% , the standerdized rate was 1. 91%. The prevalence of depressive disorders was significantly greater for female than for male. The prevalence was greater in cities than in countries. There were highly increased relative risk of depressive disorders in the oldest than in the youngest, the peak was 50 - 59 years old, and then decreased except for female in the country. Those who were divorced or losing spouse had higher odds of having depressive disorders than non - married or steady marriage. Those who were having lower education or lower income were significantlyassociated with depressive disorders. Higher education yielded lower rates, especially in the country. The same was the income. The DALYs per 1000 population were 3. 6 years and 9. 7 years due to depressive disorser for male and female in city seperately, 7. 2 and 9. 0 for male and female in country seperately. The DALYs were higher for female than for male and in city than in country. The result documented the burden of depressive disorders in liaoning was lower than that of the estimate of GBD1990 and GBD2000. This was caused by the signifance of the prevalence and disability weight.Conclusion1. The prevalence of depressive disorders in Liaoning province was 2. 24% , that was higher than the result in 1993, and this was consistant with most Asian countries, but lower than non -Asian countries.2. The prevalence was higher for female than for male. It was higher in city than in country. The prevalence of depressive disorders increased as the age.3. The DALYs were higher in city than in country, they were higher for male in city than in country, lower for female in city than in country.4. The result documented the burden of depressive disorders in Liaoning was lower than the estimate of GBD1990 and GBD2000.
