Abstract/Summary:
ObjectiveThe obsject of this study is to explore the potensial possibility of using penehyclidine hydrochloride to cure respiratory system disease by compareing respiratory machanics coefficients before and after using penehyclidine hydrochloride.Material and methodsA group 17 ASA status Ⅰ Ⅱ patients undergoing lower abdomenly surgery choosed from general surgical and gynecology who were ready for pick a day surgery without respiratory system disease history . The age of the patients were under 70. The premedication is diazepam10 mg im. without anticholinergic medicine. The operationposition is lie supine level. The induction of aneathesia medicine is fentanyl 2-3 ug/kg, propofol1. 5-2 mg/kg, vecuronium0. 1 mg/kg. Then isoflurance and N2O were been inhalaed whose flow rate was set to 1.0-1.5%.And set other ventilation coeefficient as below: flow rate of O2 was 1L/min, N2O:O2 =1:1, VT8-10ml/kg,I:E = 1:2,Vf12 - 15bpm. Artery blood sample was obtained for laboratory analysis 20 minutes after induction of aneathesia. At the same time penehyclidine hydrochloride 1mg was been injected via venous. V1.0, CT, Ppeak, Pmean were been observed and recorded each 5minutes since 5 minutes before penehyclidine hydrochloride 1mgiv.B group 12 patients choose from ICU who can not breathe by themselves need ventilation support. Midazolam 35mg and vecuronium 0. 1 mg/kg werebeen injected. Set ventilation mode to volume - control. And other ventilation coefficients were been set as below;flow rate of 02l L/min, N20:02 = 1:1, Vt 8 - 10ml/kg, I: E = 1:2, Vfl2 - 15bpm. Penehyc - lidine hydrochloride 1 mg iv. Vj 0, C, Ppeak, Pmean were been recored each 5 minutes since 5 minutes before penehyclidine hydrochloridelmg iv.Stasistical analysis was performed with theuse of the SPSS prog - ram. Analysis of variance (ANOVA) was used to compare between - group means and within - group means, Student - Newan - Keuls ( SNK) multiple co - mparisons were made to determine differences between time points withingroups at a given time. All data are reported as mean ± SD. All P value of <0.05 was considered significant.ResultsThere were no difference in compliance ( CT) , Ppeak and Pmean betw - een initial and after using penehyclidine hydrochloride within A group.But Vj 0 was higher than baseline respectively after penehyclidine hydrochloride was been injected. It was transiently increased from 86. 12% ±4. 32% to 91.59% ± 3.95% (20min)and 94.76% ±4. 15% (25min) ,and then came down.There were significantly difference in CT, Ppeak and Pmean between initial and after using penehyclidine hydrochloride within B group(P <0. 05) . CT went up from 32. 67ml/cmH2O ± 5. 23ml/cmH2O to 36. 92ml/cmH2O ± 5. 95ml/ cmH20 15min after using penehyclidine hydrochloride. VI. 0was 83. 17% ±3. 99% , then went up gradually to 89. 17% ±3. 76% 15min after using penehyclidine hydrochloride . The tendency of Ppeak was similar to that of Pmean. Ppeak descend from 16.08cmH2O ±3.60cmH2O to 13.0 cmH20 ±2. 79cmH2O,meanwhile Pmean froml5. 83cmH2O ± 3. 43cmH2O to 13. 0 cmH20 ± 2. 79cmH2O (15min) ,and then stay there.ConclusionPenehyclidine hydrochloride injection can improve respiratory machanicscoefficient of patients who had had respiratory system disease.But for those which was normal without original respiratory system disease, it can not get notable effect.
