Objective: To investigate the techniques and effects for repairing the skindefects of eyelids with advanced flaps. Methods: Since 1995, advanced flaps have been used to repair anteriorlamellar skin defects of eyelids in 28 patients. 12 patients were maleand 16 were female, with a mean age of 53 (range 15-78). Amongthem, 15 cases suffered from nevi, 8cases suffered from basal cellcarcinoma, 5 cases caused by trauma eyelid defect. All the defectranged from 1/2-3/4 length of the eyelid. Three types of advancedflaps (single-side advanced flaps, double-sides advanced flaps andA-T flaps) were designed according to the eyelid structure and defectsize and all the defects were covered by them. The incisions wereclosed with 6-0 suture. Results: All the flaps were survived with primary healing except for onecase of distal epidermal necrosis. Follow-up for 3 to 12 monthsshowed that the total choiceness rate is 85.7%, all the cases achievegood appearance and function. No eyelid retraction, ectropion andother severe secondary deformities were found. Conclusion: 1. Using the advanced flap to repair the severe skin defect ofeyelid is a simple and effective method.2. The color and the quality of the skin in the donor site aresimilar to those of the recipient site.3. The blood supply of the advanced flap is reliable. It canprovide satisfactory cosmetic effects and normal functions withoutsevere complications.4. It is a one-stage reconstruction.
