| CGN is the familiar clinical symptom in urinary system disease, the CGNin primary glomerular disease, the clinical situation is proteinuria, blood urine,hypertension, dropsy.There is no certain therapy with west medicine, and thepathogenic damage of some patients is ongoing, will result in renal failurefinally. CGN belongs to blood urine, waist pain, and emptiness syndrome, ect.The clinical symptoms of primary glomerular hematuria are different accordingto the different pathogenic factors. Therefore, treatment determination based onsyndrome differentiation can accept good effect. According to the clinicalcharacters, many patients have the blood stasis syndrome in TCM, and bloodstasis can be an independence syndrome , also an accompanied symptom inother syndromes. This research set up contraposing the clinical effect, based onthe wholism conception and treatment determination based on syndromedifferentiation in TCM, the principle is eliminating blood stasis , regulatingblood circulation, clearing heat and detoxifying. The research sets up thesystemic clinical observation and study in CGN using purifying kidneydecoction, and aims at the clinical effect and approach the mechanism of thisdecoction.There are 78 cases in this research, all from TCM department and renalmedical department of China-Japan Union hospital of Jilin University, the maleare 42 cases, and the females are 36 , the age of them is from 18-63, dividedinto therapy group and control group in random, and there is 40 cases in therapygroup, 38 in control one. All of the cases are diagnosed as primary glomerularhematuria according to the , and ,and differentiated syndromes according to ,, and all the patients haven't had correlative drugs in last 2weeks. There are four differentiations of symptoms and signs for classificationof syndrome, the Jade Screen Powder and Astragali seu Hedysari Decoction isused in dideficiency type of QI of the lung and kidney;Shipi Decoction is usedin deficiency of spleen-YANG one of asdthenic splenonephro-yang;JishengShenqi Decoction is used in deficiency of kidney-YANG one;RehmanniaeDecoction of Six Ingredients is used in hepatic and renal yin deficiency;ShenqiDihuang Decoction is used in type of deficiency of both QI and YIN. Therapygroup adds followed drug for invigorating blood circulation and eliminatingstasis: danshen root,motherwort,safflower, prepared RADIX ET RHIZOMARHEI with wine,chuanxiong, and cyathulae ( all Chinese herbs are decocted inTCM pharmacy of China-Japan Union hospital ). Therapy course: 4 weeks as aone course, 2-4 courses consecutively.The result: â‘ The patients' urinalysis BLD, RBC decreased distinctly intherapy group after taking purifying kidney decoction, dealt withstatistics( P<0.01), has the difference significance. â‘¡The RBC elimination ratedeclined, and the abnormal red blood cells decreased in therapy group aftertaking the decoction, dealt with statistics( P<0.01) , has the differencesignificance. â‘¢The decoction can rectify the low serous alexin syndrome, andmodulate immunoglobin in therapy group, dealt with statistics( P<0.01), has thedifference significance. â‘£The can also decrease the higher CRE and BUN,improve renal function, dealt with statistics( P<0.01, P<0.05 ), has the differencesignificance.The drug for invigorating blood circulation and eliminating stasis takes thetheory of TCM as the basic principle, has the effect of eliminating blood stasis,regulating blood circulation, clearing heat and detoxifying. In the decoction, allthe herbs supplement each other to acquire perfect clinical effect.In my topic,danshen root,motherwort,chuanxiong,safflower, prepared radixet rhizoma rhei with wine,chuanxiong, and cyathulae.these Chinese herbs in thisdecoction has the following effect to treat hematuria:1.Promote the generation of coagulant factors, enhance the concentrationand movement of coagulant factors.2.Increase the quantity of platelet, and improve the function of platelet.3.Contract the local vessels and improve the function of the vessels,strengthen the resistance of capillary vessels, decrease the penetration of thevessels.4.Promote the generation of fibrinogen or fibrin, restrain fibril dissolution.5.Others.The drug for invigorating blood circulation and eliminating stasis also hasthe immune regulating function, and this kind of function is a bidirectional one.The decoction can can rectify the low serous alexin syndrome, and modulateimmunoglobin in therapy group, may improve the red blood cell's immunity.The mechanism of the immune regulating function is not clear in the research,will be studied more in the future.Generally speaking, the method of eliminating blood stasis and regulatingblood stasis, in treating CGN, has brought more and more attention, andprovides a new way to treat this kind of hematuria.
