The Contrasting Analysis Between EST Combined With LC And Open Cholecystectomy With Choledocholithotomy In Treating Cholecystocholedocholithiasis

Objective: To compare the effect of Endoscopic Sphincterotomy(EST)Combined Laparoscopic Cholecystectomy(LC) with open cholecystectomy andopen choledocholithotimy T-tube drainage (OC+OCHTD) to evaluate the value ofEST + LC for the treatment of CholecystocholedocholithiasisMethods: A retrospective study was adopted to analyze comparatively theclinical date of 77 patients of Cholecystocholedocholithiasis treated with differentways. 23 patients were treated by EST+LC, and 54 patients were treated byOC+OCHTD. The clinical date of two groups was contrasted and undermentstatistical analyze.Results: There were no statistical difference in the success rate and postoperative complication rate between two groups, but the post operative compliationof the EST+LC group is less serious than that of the OC+OCHTD group. Therewere statistical difference in the recovery time of gastroenteric function and thehospital stay .The cost of two groups have no statistical difference, although theaverage cost of the EST+LC groups is less than that of the OC+OCHTD groups. 42patients received regular follow-up for 5-25 months(mean 14.6 months), none ofwhom was found common bile duct recurrence and cholangitis.Conclusions: Compared with traditional open cholecystectomy and opencholedocholithotimy T-tube drainage, EST+LC has the virtue of litter trauma, fastrecovery, short hospitalization days and short recovery time of gastroentericfunction. Although the success rate and the post operative compliation have nostatistical difference, but the post operative compliation of the EST+LC is lessserious, and the cost of the EST+LC didn't increase. In a word, EST+LC is asafe ,reliable, feasible and less invasive proceduce for the treatment ofCholecystocholedocholithiasis.
