Objective To evaluate application of transvaginal ultrasound (TVS) and color Doppler energy (CDE) in diagnosis on small ovarian masses . Methods Eighty-two patients with 94 small ovarian masses were studied prospectively with TVS and CDE, including 33 functional cysts, 50 benign masses and 11 malignant masses . The results were compared with the pathological diagnosis or the followed -up regression within six months after operation of the small adnexal masses. Results there were 4 malignant masses in stage one and 2 bordline ovarian masses of 11 small malignant ovarian masses. The rate of diagnosing early ovarian lesions was 54.5% with TVS and CDE. The Sassone ultrasound score was less than nine in one malignant mass and solid portions of the malignant tumors were detected lower resistant arterial flow with CDE in only 5 masses. The Sensitivity, Positive Predictive Value, Positive Likelihood Ratio of hemorrhagic corpora luteum, mature cystic teratomas , endometriomas and small malignant masses were 80.00%, 94.12%, 59.20; 95.24%, 95.23%, 69.53;82.35%, 77.78%, 15.85; 81.82%, 56.25%, 9.70;respectively. Conclusions TVS combining with CDE appears more predictive value in hemorrhagic corpora luteum and mature cystic teratomas. With a thorough knowledge of the pathomorphological changes associated with the different disorders of ovary , early diagnosis rate of malignant ovarian masses can be improved by CDE and multiple clinical techniques on the basis of transvaginal ultrasonographic examination.
