Objective To study primarily the influence of acute hypervolemic hemodilution (AHHD) on economy's secure time in anoxic status of general anesthesia. Methods Sixteen patients (aged 20-40 years, weight 50-70kg), ASAI, scheduled for operations were randomized into two groups: Control group (n=8) and AHHD group (n=8). Both groups were received basic liquid requirement using Lactated Ringer's solution (10ml/kg) before intubation. After intubation, the AHHD group was performed AHHD by 6% HES (200/0.5) 15ml/kg and Lactated Ringer's solution 8 ml·kg-1·h-1 synchronously. The control group was only infused by Lactated Ringer's solution 8 ml·kg-1·h-1. After 40 minutes, the oxygen provision of Anesthetic machine was shut off, recording the secure time in anoxic status (SpO2 from 100% down to 90%). Arterial blood samples were taken at intervals before induction, ventilation 40 minutes, and the end of SpO2 from 100% down to 90%, to analyze the blood gas. Results The AHHD group:before hemodilution Hct(%):37.4±4.7, Hb(g/L):128.8 ±15.5, after hemodilution Hct :28.8±2.8, Hb: 101.3±9.9. Compared with basic levels, after AHHD Hct and Hb dropped by 23.1% and 21.4% respectively. The secure time in anoxic status of the control group was 563.4±168.6S, while that of the AHHD group was 373.4±97.9S. The difference of the secure time had no statistical significance, as well as the difference of SpO2, SaO2, PaO2, PaCO2, PH of both groups at the sametime had no statistical significance. Conclusions Compared with the control group, the moderate AHHD had no influence on the Secure time in anoxic status. It indicated that moderate AHHD did not decrease the organism endurance in anoxic status. It was safe that AHHD was an important technique to economize blood in clinic. This study deduced the secure time range to correct the anoxic status for maintaining the normal SpC>2 in the clinical anesthesia, which is 6-10min.
