Objective To evaluate the immediate and follow-up results and complication ofpercutaneous transluminal septal myocardial ablation ( PTSMA) in patients with hype-rtrophic obstructive cardiomyopathy (HOCM).Methods 12 patients with HOCM refractory to medication was accepted PTSMAThemethods of target septal branch selection included balloon occlusion test and left ventricularoutflow tract pressure gradient (LVOTG) monitor. The immediate effect was evaluated byLVOTG monitor.Resuts The successful rate of PTSMA operation was 100 %. Immediately afterPTSMA, the LVOTG reduced from 30-120 (89.7 ±22.4) mm Hg to 0-20 (7.6 ± 8.2)mmHg at rest (P < 0.001) and from 103-180(152.2 ±23.5) mm Hg to 20~70mm Hg afterextrasystole (P<0.001) .Tested by catheter, LVOTG reduced 57 %-100 %. Complete rightbundle branch block (CRBB) was found in 11 patients and all of them were permanent.Nosevere arrhythmia occurred. During a mean of 3-6 months follow-up, the clinical symptomand exercise endurance of patients were improved.Conclusion PTSMA is a effectively non-surgical trentment method for HOCM. Theoperation will be safe when the case and target vessel are chosen properly and with accumulated clinical experiences.
