Objective To study the probably protective mechanisim of moderate hypothermia through investigating the changes of SIOOB protein and IL-1βconcentration in CSF of patients after extensive contusion of brain and the effect of hypothermia on S100B protein and IL-1βconcentration.Methods Fifty-six patients with extensive contusion of brain were divided into hypothermic(HT) groups (27 cases)and normothermic(NT) groups(29cases). All of the patients were placed an intraventricular catheter respectively and CSF samples were obtained at 6h,24h,3d,7d after injury, the levels of ICP were recorded during the treatment. CSF SIOOB protein and IL-1βconcentration were measured by enzyme-linked immunosorbent assay. To compared with 10 healthy adults (the control group) in order to study the changes of S100B protein and IL-1βcontents in CSF.A11 of the pationts with a Glasgow Coma Scale score of 8 or less on admission were included in this study.Outcomes of all pationts were evaluated by GOS 3 months after injury. The prognosis were classed as death,favorable and unfavorable .Result (1)All the concentration of CSF S100B protein and IL1βin patients were higher than that of the control group markedly(P<0.01).②the CSF S100B protein concentration in HT groups were significantly lower than that in NT groups at 24hour,3day and 7day time-point, the CSF IL-1βconcentration in HT groups was significant lower than that in NT groups at 3 day and 7day time-point.③The favorable rate in HT groups were significantly higher than that in NT groups (x~2=4.624, P=0.032), The death rate in HT groups were significantly lower than that in NT groups (x~2=4.835, P=0.028).④Significant positive correlation was found between CSF S100B protein and IL1βin both groups at the every following time-point.⑤There were no significant differences in ICP between HT groups and NT groups at 6 hours after injury. The ICP in HT groups were significantly lower than that in NT groups at 24h,3d and 7day point.⑥A positive correlation was found between CSF S100B protein and ICP at each sampling point. A positive correlation was found between CSF 1L-1βand ICP at each sampling point.Condusion The+concentration of CSF S100B protein and IL-1βwere elevated in all patients after extensive contusion of brain. S100B protein and IL-1βmay be played a important role in the secondary brain injury and participate in inflammatory response. The treament of moderate hypothermia can significantly suppress the CSF S100B protein and IL-1βconcentration in patients after extensive contusion of brain,reduce the level of the ICP, protect brain tissues, depress death rate and improve the clinical prognosis.
