Owing to plentiful lymphatic tissue,neck metastasis iscommon in supraglottic carcinoma.So when the primarylesion is excised,the treatment of the neck is usuallynecessary.As a result,the accurate prediction of the necklymph node before procedure is very important.To observe MMP-2 and E-cadherin expression and theirrelationship with lymphatic metastasis in supraglotticcarcinoma,48 cases were detected using immunohistochemistry.1. MMP-2 expression and its relationship with lymphaticmetastasis in patients with supraglottic carcinoma10 cases of normal mucosa and 48 cases of supraglotticcancer were detected, no expression was observed in normalmucosa . MMP-2 expression rate was 66.7% in tumor tissue,it was immunolocalized on the cytoplasm.Strong staining ofMMP-2 was observed in the front of the tumor,indicatingtheir strong invasive ability through the degradation oftype â…£ collagen.Compared with group without lymph nodemeatstasis the MMP-2 expression was much higher in groupwith lymph node metastasis.MMP-2 play a key role in theinvasion and metastasis of supraglottic carcinoma oflarynx.2.E-cadherin expression and its relationship with lymphnode metastasis in supraglottic carcinoma.10 cases of normal mucosa and 48 cases of supraglotticcancer were detected,E-cadherin was strongly stained inall the 10 normal laryngeal mucosa. positive expressionrate of E-cadherin was 52.1% in tumor tissue.It wasimmunolocalized on the membrane.In tumor tissue withstrong staining of E-cadherin,tumor cells infiltrate thenormal tissue in the form of mass,while cluster or diffusetumor cells in tumor tissue with poor or no expressin ofE-cadherin.We speculate that downregulation of E-cadherinresults with the downregulation of the adhesive abilitybetween tumor cells,thus these tumor cells can escape theprimary lesion and metasize to the lymph vesseleasily.Compared with group with lymph node metastasisE-cadherin expression rate was much higher in group withoutlymph node metastasis.It can be speculated that thedownregulation or absence of E-cadherin was significantlyrelated with the lymph node metastasis of supragoltticcarcinoma.3.The correlation between the expression of MMP-2 andE-cadherin in supraglottic carcinomaMMP-2 and E-cadherin was negatively correlated insupraglottic carcinoma.4.MMP-2,E-cadherin expression and the prediction of lymphnode metastasis of supraglottic carcinomaIn our test, we predicted neck metastasis usingMMP-2(+),E-cadherin(-),and MMP-2(+) with E-cadherin(-) asindex repectively.The result showed that MMP-2(+) had thehighest sensitivity,but lowest specificity and positivepredictive value.On the contrary, MMP-2(+) withE-cadherin(-) had the lowest sensitivity but highestspecificity and positive predictive value. E-cadherin(-)was intermediate.Acted as two important proteins in theinvasion and metastasis of the tumor,MMP-2 and E-cadherinare reliable markers for the prediction of lymph nodemetastasis of supraglottic carcinoma.Combined detectionof MMP-2 with that of E-cadherin,we can boost the accuracyof the prediction of neck lymph node metastasis insupraglottic carcinoma.
