Comparing The Effects To Proximal Humeral Fracture Fixed By Anatomy Plate, T Shape Plate And Clover Plate Respectively

Goal:To compare the advantage and disadvantage of differentinternal fixation to proximal humeral 2,3 and 4 parts fracturesaccording to Neer classification fixed by anatomy plate, T shapeplate and clover plate respectively.Method:60 random samples were selected from the patientsof proximal humeral fracture cured in 3rd affiliated hospital of JilinUniversity from Jan,1998 to Dec,2002.Each group include 15 casesrespectively. Results: 48 random samples are called back to. Tshape plate(15 cases):The calling back time ranged from 5 to 28months and the average is 13.5 months.The healing time rangedfrom 3 to 7 months and the average is 5.5 months. Clover plate(17cases):The calling back time ranged from 6 to 25months and theaverage time is 17months.The healing time ranged from 8 to 25months.Anatomy plate(16 cases):The calling back time ranged from3 to 23months and the average time is 15months.The healing timeranged from 7 to 19 months and the average is 2.5 months. All casesincluded healed without infection, disconnection and necrosis ofhumeral head. Evaluation of the cases' shoulders'function isaccording to the Neer's shoulder function classic system[1].Result: Evaluation of these cases' shoulders'function isaccording to the Neer's shoulder function classification standard.Thefunction score:without limitation and unfit during working andmovement is 30 points.Slight limitation and a little unfit is 20points.Middle degree limitation and unfit is 10 points.Significantlimitation and unfit is 0 points.The movement range of shoulder jointevaluation:Adding to 7 kinds of movement degree to get a score(Thenormal is about 500 degree).20 degree are equal to 1 point(Thenormal is about 25 points).The evaluation of anatomicalposition:Anatomical replacement is 10 points.Malformation is 0points.All the 4 evaluation mentioned above add up to get ascore.Higher than 90 points is excellent.From 70 to 89 is fine.From60 to 69 is normal.Less than 60 is bad. The excellent rate of casesfixed by T shape plate, clover plate and anatomy plate arerespectively 86.7%, 88.2% and 87.5%.conclusion:Comparing the 3 kinds of plate mentioned above,we can draw the conclusion that T shape plate is more suitable to 2and 3 parts proximal humeral fracture and not suitable to 4 partfracture. Clover plate and anatomy plate are suitable to 2,3 and 4parts frature.As to 4 parts fracture, the shoulder joint' functionhealing rates of cases cured by clover plate is higher than the other 2kinds of plates.The 2 cases cured by T shape plate whose shoulderjoint function evaluation is bad are both senile patient and theirfracture is 4 part.It shows that the therapy effect is connected topatient's age,trauma degree, fracture kind, gender and basic physiccondition.
