Objective: To investigate the effect of acute hypervolemic hemodilu-tion (AHHD) on the patients and clinical security and feasibility by using the double variable parameters formula. Methods: Thirty two patients, ASA I — II, aged 20-60, weighted 45-70kg, scheduled for otolaryngocal surgery, were divided into two groups randomly: The AHHD group (n=16) and the control group (n=16). AHHD was performed using the double variable parameters formula (the perfusion volume = [ (H0-Hf) /Hf]× weight(kg) × 7% ×1000) after general anesthesia. Arterial and venous blood samples were taken for determination of Hct, Hb, liver function, kidney function, coagulation function, PaO2, SaO2, PvO2, SvO2, BE and lactic acid before and after AHHD, during and at the end of the operation. CaO2, C vO2, ERO2 were calculated by special formula. Results: Compared with basic data, after AHHD Hb and Hct dropped by 34.5% and 32.6%, and reached to the target Hb (90g/l), Hct (26%) respectively. CaO2 and CvO2 also dropped by 20.7% and 20.5% respectively, which were still several times higher than normal levels and the difference of which were statistically significant. Compared with the control group, ERO2, SvO2, Lactic acid, T, BE and pH of the AHHD group dropped slightly and without statistical difference. Conclusions: After moderate AHHD, the cardiac and pulmonary functions remain stable state, the changes of MAP and CVP are remained within the normal range, and the Lactic acid and ERO2 are not significantly affected. Therefore, According to the double variable parameters formula were performed AHHD which is feasibility and security in clinical practice.
